I think there is a rat cosmetic you get from having all the rat related perks, you can use Cheatgui if you can't get them in a regular run.

there's a bunch of other cosmetics u can get from having I think 3 perks, there's a spider one (lukki mutation and lukki minion with either leggy mutation or a perk duplicate), a halo (saving grace, more love, extra life, peace with gods i think with a total of 3 (any combo)) devil halo (exploding corpses, more hate, and another one I can't remember), mushroom head (cordyceps, fungal spores and one other one) plus an amulet you get from completing a secrete, a gem for said amulet by completing one of the hardest secrets, and a crown for completing a secret questline that involves laser eyeballs and taking celestial bodies for walks, and murder.
