it's really random. There are some youtube videos on this bug, like the archer in the first tree platform after you find Hyo's tomb in the campaign level "infiltrating Kyuryu" (or whatever it is)

Yes, I'm aware. It's a bug in the game. Sometimes they will get alarmed even when you're hiding behind something and they have their back towards you.

After 30 something attempts I think I found the best route through love it very challenging with no alert and no kills.
