I tinkered with it so I'd get the effects on low health, and it worked, but at some point the visual effects aside from screen throb disappeared and the glitches only appear on death.

It'd be nice if there was a restore to default button.

I think I pushed an update before steam started returning Generic Failure's for me.
Open Q-Menu, go to Options(or Utilities, i don't remember) -> Threat Effects -> Death Effect Options

Here's how.

Go to add-ons, hover over the mod and hit disable. It's the best fix for the moment.
