Check SteamApps\common\Left 4 Dead 2\left4dead2\addons\workshop - maybe the mod stayed there after unsubbing? Never happened to me before so I don't know if that's the way to fix it.

I tried out a new compatible skin and unsubscribed because colnel nick's face stayed after I unchecked it in my addons, but it still won't go away. (your mod's awesome, but I just wanted something newer, so no hard feelings, ok?)
