Reworked commander call-in vehicles & units for West & East Germans.

Vehicles can now be requested to map co-ordinates where often before they just appeared in the build queue.

v2.31 Added the new bunkers in, replacing my instant build ones. They are a mix of the new HMG bunker & with my previous bunker spawn options.
Removed an option from the Puma so it can hull down, wasn't always showing.
Removed Elite Tiger skin, will use standard tiger now, for those that use custom skins.
