Removed the minus crit chance mods as they did not actually work at all!

Also fixed the No Furnaces mod! This still needs some tweaking, but it works!

Made a more thorough attempt to fix the No Furnaces fountain mod.

* Fixed the No Shops and No Elevators wishes not showing up!

Removed the No Furnaces wish, because it wasn't working.
This wish existed in the vanilla version of the game, but it was commented out. Now I see why. (:

Set the 1 HP wish to be usable at fountain level 1. This way any masochist that wants to attempt this on a fresh new profile can embrace the pain right away.
