Big code refactor, but also compatibility with Last Judgement

Crashing after spawning the chests won't deny you them after reloading, Glowing Hourglass will now work properly with the chest spawns, all methods of visiting the floor twice in one run (Forget Me Now, R Key, victory laps) will now properly respawn the chests.

Now works in Red Redemption!

Fixed chests sometimes spawning as mega chests, which aren't guaranteed to give items.

Damocles no longer doubles items spawned from the chests.
