corrected http synchronization which failed previously due to being executed before the gmod server was ready

made mapvote start quicker, corrected localizations

+ made missing map icons be downloaded from a server (no need for map icon packs anymore; more icons will be added soon; also Super Map Icon Maker addon will receive feature for uploading your own map icons to the server)
+ added global ratings that are synchronized across all servers having this addon
+ removed unnecessary config options
+ removed images and stars from mapvote's 'list layout' (now reserved for the 'image layout')
+ removed workshop downloads of map previews and replaced with requests to central server instead
+ made rating gamemodes and maps one step instead of two
+ fixed icon displacement when AdminsHaveMoreVotePower is enabled
+ made more global functions local
+ improved !mvmenu element scaling
+ reduced unnecessary file checks at startup, therefore increased performance
+ fixed bug where unassigning a blank value from gamemode prefixes would fail

making survey whether or not map had missing css textures only for servers where css maps are disabled, fixing map ratings with 1 count 0 stars
