Fixed SetWeaponHoldType errors

Fixed C4/IED/MINE so they remove when a player disconnects.

This fixes the infinite explosions glitch with C4.
It also prevents the possibility of that with the IED/MINE.

Thanks to Darkaaja for implementing these fixes and testing with me.
