Step 1:

Open "Steam\SteamApps\common\Left 4 Dead 2\bin" and right click "vpk.exe" send to desktop.

Step 2:

Now copy/paste "Schism Hud" from "SteamApps\common\Left 4 Dead 2\addons\workshop" to the desktop.

Step 3:

Drag the mod onto the "vpk.exe-shortcut" to unpack the mod.

Rename this folder to anything you want. Then open the folder and head to the [extras] folder.

Step 4:

Select your Options in the [Extras] folder.

Step 5:

Back in the main directory Paste and Overwrite when asked. (repeat as necessary)

Step 6:

Lastly change the name of the mod by opening "addoninfo.txt".

Doing this will make it easier to find later in game. Save.

Step 7:

Drag the Mod folder onto the vpk.exe-shortcut to repackage the mod.

Step 8:

Drop your New mod into the "Steam\SteamApps\common\Left 4 Dead 2\addons" folder and enjoy!

You need L4D2 Authoring Tools.
Located in the "tools" section of the steam library.

my game has no vpk.exe in my bin folder

Go to the scripts folder and open hudlayout.res in notepad (preferably notepad++) and delete the Letterbox entry (lines 15-29 in ++). Just be aware that version of the hud was designed for letterboxing.
