-Do not talk to Nona at Maggie Adler's house (The completetionist in me wanted me to talk to everybody and interact with all the things)
-Do not find all the items at Maggie Adler's (The completetionist in me wanted me to talk to everybody and interact with all the things)
-Do not talk to Nona at Bridge Stand during the time loop (The completetionist in me wanted me to talk to everybody and interact with all the things)
-Close the gate when you are underwater (I called out a name instead)
I think the deciding factors for the achievement are just choosing all the "fire" dialogue choices and trading Clarissa.
-Do not accuse Ren of lying during Truth or Slap (I accused him for the achievement)
-Go help Ren first after visiting the Comm Tower (I went to Clarissa first based on Jonas's advice)
- Get all the answers right in the army base (US Army RCS, Cookies, 85) (It was a blind playthrough, so I got 2/3)
-Take Jonas with you to Main Street (I took Nona since the other two were arguing)
-Help Jonas with the tape player inside the bunker (I thought it was a trap)
-Close the gate when you are underwater (rather than walking away) (I did not know about that option.)
