Inside the lobby there is a red valve with a spotlight above it (left side of the screenshot). Teleport/stand close to it and it will start spinning and you will get a message that you have unlocked the headcrab head (right side of the screenshot).

You can now use it by pressing the menu button and picking it so that others will see you as a ball with a headcrab shape on it.
