Does your game bug out like this? Here is a fix!

Open console, if it doesn't open, check if it's enabled.
(Options>Advanced>Enable Developer Console)

Type :
r_eyesize 0
Then reload the map.

If that filthy bug comes back after you restart, add it to the autoexec.cfg.
Here is where autoexec.cfg located at:
(Garry's Mod installation folder)>garrysmod>cfg>autoexec.cfg
Open it and add those lines:
r_eyesize 0
echo "autoexec.cfg executed!" (to know if autoexec is executed, this text will be written on console)
Now you don't have to write this command everytime you open the game!
