Convert the musical file in a .wav format at 16-bit, 44100 Hz, with either Mono or Surround.
Create a folder in Steam/steamapps/common/Left 4 Dead 2/left4dead2/addons that is the same of the file you want to change. (Ex. wanna change themonsterswithin.wav? Create in addons a folder with a custom name that contains sub-folders: Customname/sound/music/unalive and place the file with the right name inside the folder.
If you want to publish the file on the workshop, you should also create an addoninfo.txt file which looks like this:
