⁺₊✧☾ Hi! I wrote this guide after getting frustrated with the game. It's specifically for the Autopsy Only Mode achievements - for my fellow morbid-obsessed folks who want to max out this game mode even with how tedious it is. I felt like there isn't a definitive guide for just this mode and some achievements are rough so this is how I personally got the 100%!. For story mode achievements, there's other great guides you can check out. ☽⁺₊✧

DISCLAIMER: This is things I've personally noticed in my own gameplay. I'm not a developer of the game nor do I have any additional insight other than getting the achievements myself and having almost 20 hours in game. Have in mind that all of these achievements are secret on Steam so there might be spoilers and/or fun ruiners ahead.

If you're determined to get all of them, I recommend aiming to get The Perfectionist from the beginning and all the rest (with the exception of Speed Surgeon) will just pop for you in the meanwhile because you need a lot of runs to get The Perfectionist.

Complete your first autopsy.

This achievement should pop up when you submit the cause and manner of death in your first autopsy.

Solve the alligator bite mark case in AOM

For this one you need to solve any case with an alligator bite mark present on the body. Make sure to put the bite picture into the fax machine, trace it and match it with a picture from the book. The achievement will pop up when you match it correctly.

Make 100 cuts with the scalpel in AOM

No worries, you don't need to cut open 100 bodies for this one. For some reason opening a body with a scalpel counts as multiple cuts (I assume every slushy/cut sound counts as a cut). This one popped for me pretty fast, it should be done after a few bodies.

Dissect 15 organs on the cutting board in AOM

There's 4 organs you can cut on the cutting board in each body: the lungs, liver, stomach and brain. If you dissect all of them, even the healthy ones, it should be done in 4 runs. As far as I can tell, the trachea doesn't count for this achievement as it's cut on the tray instead of the board. This one popped for me instantly after finishing the labirynth mini game for the 15th time.

Capture clear, detailed evidence photos 50 times in AOM

This one takes a while, but it will just happen naturally if you're going for the 100%. You don't need to do anything specific, just take any and all pictures the game lets you of each body.

Examine 50 vital organs accurately in AOM

Examining in this context is just taking the organ out of the body and letting the note be written down. The heart and intestines count towards this one as well.

Identify 50 injuries with a magnifier in AOM

Any injuries you use the magnifier tool on count towards this one so make sure to magnify all of them on each body.

Handle every type of case at least once.

As each case is randomised, this one is one of the longer ones to fulfill, but you'll get it eventually if you keep playing. As far as I can tell, "type of case" refers to all causes and manners of death, so 8 causes and 5 manners, 13 in total, not all combinations of them. You don't have to guess the cause and manner of death correctly for the cases to count towards this achievement.

Achieve 100% score in autopsy report

You need to get an A+ grade on an autopsy to get this one. I messed up on one minigame labirynth and still got it, but be careful to not make more than one mistake in the minigames because it can drop your grade to A or lower.

Planning/Preparation:

• Put on the gloves and apron
  
• Grab the camera
  
• Go through the police file (make sure to click on all 3 pictures)
  
• Unzip the bag and take all available pictures. You'll know you've taken all pictures when there's a click sound cue and a plus pops up on the bottom right on the screen + you can't equip the camera anymore after putting it down.
• Put up all pictures on the magnetic board (if there's an aligator bite, you need to trace it and match it at the desk, then put it up on the board)
  
• Perform the rigor test
  
• Use the magnifier on all wounds and marks on the body
  
• Optionally you can use the UV light on the whole body

Minigame score:

• Dissecting at least 2 organs correctly, the rigor test and cutting the ribs should amount to a 100% ,but you can also perform the centrifuge blood test and cut open the head and more organs to maximise minigame points.

Cause of death guess:

• The correct cause of death amounts to 72% of the score and correct manner of death amounts to 28%. See additional tips in this guide for some constants I noticed in my games if you have a difficulty to guess both correctly.

Cleanup score:

• This is just about closing the skin flaps and sewing the body up completely, you don't need to do anything else to get 100% on clean up. To not forget about this step, I recommend doing this immediately after you've examined all organs of interest.

note: the number of available points is different on each case, so pay attention only to the percentage

Complete an autopsy of score C or higher in under 3 minutes.

This is essentialy a speed run + having luck in guessing, so it will take a few tries. I recommend doing this one after you've already got a grip on what to do and can do it in just a few clicks, I also recommend starting a timer and starting over if any steps takes too long because there's too many outside injuries. Personally when I saw the alligator bite on a speed run, I started over because to put the picture of the bite up you need to scan it etc and it takes a bit too long.
To get this one you need 100% on preparation and 100% on cleanup + a correctly guessed cause of death. Anything over 00:03:00 won't count and closing the freezer counts to the time as well.

How to do an autopsy in under 3 minutes:

• Put on an apron and gloves instantly after loading into the game, then grab the camera
  
• Get the police report and click through all the 3 pictures as fast as possible
  
• Unzip the body bag and start snaping pictures, spamming the back button (right click) after the shutter sound plays
  
• When you make sure you've taken all the pictures (the plus pops up), go up to the magnetic board and put up all of them
  
• Go up to the body and use the magnifier tool on all of the wounds, spamming the back button again to try to skip the writing animation (it doesn't always work but it spares some time)
  
• Use the scalpel to cut open the body and immediately use the needle to close it (no need to open the skin flaps)
  
• Guess the manner and cause of death (you can skim through the file if you have some time to spare but it's just a blind guess anyways) and place a dot or a quick line on the signature spot
  
• Remember to instantly close the doors to the freezer after guessing cause of death

Perform 25 flawless autopsies.

This specific achievement is the reason why I decided to write this guide to spare anyone from my pain if you're going for the 100%.
I tracked my grades when I was trying to get this one and figured out that you don't need to get 100% (A+) on all 25 runs, but you need 25 of A and up. I got this one after getting A+ 12 times and an A 13 times - for an A you need to have a perfect score on preparation, clean up, cause and manner of death, the only place you can spare some points is mini games - so no need to give up if you're grinding for this achievement and mess up the mini game labirynth once or twice.
Getting this one took me personally about 10-12 hours of gameplay and other players who I've seen have it, have from 12-20 hours of gameplay as well. Have in mind that the cases get pretty repetitive so you have to be pretty determined to get this one.

1. If some achievements you know you've done don't pop up for you, close the game, wait til it uploads to the cloud and open it again.

2. The aligator bite mark in AOM is always the same exact one (Mississipi Aligator), just located on different pages in the book.

3. A bug with picture taking on the woman with bangs:
Sometimes on that cadaver, it won't let you take a picture of a bruise on the forehead, unless you position it above her actual face:




4. Guesses of cause and manner of death (these are the deductive connections which were almost foolproof for me):

• burns on hands and feet + charred heart with thermal damage + edema in lungs = cause of death is electrocution
  
• snake bite mark + cyanide in the centrifuge blood test + mention of an out of country trip or animal wrangling in the police file = cause of death is snake bite
  
• darkened color and soot in lungs and trachea + tumor markers in the microscope blood test = cause of death is lung cancer (which always has natural as manner of death)
  
• pills in stomach or needle mark on body + cyanide or heroin in the blood test = cause of death is chemical poisoning
  
• infected wound on body + bacteria in the blood test + mentions of contaminated water or unsanitary living conditions in the file = cause of death is sepsis
  
• rashes on the body + any pathenogenic signs on the trachea and lungs + mentions of allergies in the police files = cause of death is anaphylaxis
  
• the examination notes on the heart mentioning the signs of a heart attack = cause of death is heart attack (this worked for me even if other causes of death like chemical poisoning made more sense)
  
• if the police file mentions someone suspicious leaving the crime scene - manner of death is always homicide
  
• if the police file mentions that police suspect deliberate harm or that the deceased was depressed - manner of death is always suicide
  
• if nothing mentioned above is in the police file or "police don't suspect foul play" is mentioned - the manner of death is usually an accident (unless it's lung cancer), it was very rarely "unknown", if ever.
