This list is a rearranged version of my other list, meant to be used from a New Game Plus run having beaten the game once before. This is not meant to help you find the few items you're missing, this is meant to be used as a one-stop shop through the whole game to keep you on track. You can always go in a different order if you like, this is just the order I decided I liked most.

Part 1
Upgrade #1: Winged Boots
Location: Vann’s Point
Prerequisites: Bring dinner to Pa

After waking up in your new home, you’ll be sent out by Bolivar to go get your pickaxe, but there’s a fetch quest that can be done first for one of the most important items in the game. It’s a chain of “talk to this NPC, they’ll ask you for something from someone else, that person also needs something” repeating until you finally get the item, then you walk it back. The order is as such; Tara, Nora, Lynn, Tess, Ashwood’s Maid, Quinlan, Mariana, Hugh, and finally Ashwood’s Butler. Walk that line back to Tara and she’ll give you the Winged Boots, which can be equipped via Tess’ shop.

Location: Vann’s Point
Prerequisites: Pickaxe

Found next to River’s house in eastern Vann’s Point, near the lake.

Coin of Crowl #1
Location: Boneyard
Prerequisites: Pickaxe

In the third room with the big roller, use the Pickaxe to break a hole in the left wall to open a secret room with a few chests, one of which holds this special Coin. It has no monetary value, but will come in handy later.

Heart Ore #2
Location: Boneyard
Prerequisites: Dread Hand

Found on the left side of the room with the boss door. Lower the blocks to grab it.

Heart Ore #3
Location: Tidal Mines
Prerequisites: Lariat

Once you obtain the Lariat, backtrack to the third room of the dungeon (2 up from the entrance) and use the Lariat to reach the platform in the upper right of the room, leading to a puzzle with a Heart Ore as a reward

Coin of Crowl #2
Location: Tidal Mines
Prerequisites: Lariat

In the room where you’re meant to use the Lariat for the first time to progress, first use it to reach the small platform on the eastern side of the room. It looks pointless at first, but knock a hole in the wall to access a secret room with another coin in a chest

Heart Ore #4
Location: Tidal Mines
Prerequisites: Lariat

From the boss door, grab the key to the south and then head right a room from the door again and use the Lariat to cross the pits to the bottom right side where the ore resides

Upgrade #2: Hearth Ring
Location: Vann’s Point
Prerequisites: Clear Tidal Mines

With the Lariat in hand, you can now play River’s fishing minigame by investigating the journal in her house. You have infinite time to fish, so just keep catching fish until you reach the cap in score, which will make Keaton give you this ring as a prize when you go see her. The side effect of this is no longer having to pay for the Smoked Salmon from Keaton, which is a massive win. Silver ones will lower your score, so avoid them

Location: Vann’s Point
Prerequisites: Clear the Tidal Mine

Pick up the harmonica from your bedroom then head back into the Tidal Mine and go right from the second room to find a weird rock. Play the harmonica for it to cause it to move and access Zaegul’s Secret Shop, where a ton of cool items are, but we’re here specifically for the rare seeds. You may have grabbed a few up to this point, but make sure you have 6 in your inventory and plant them in all the tree stumps around Vann’s Point. This will cause Aoife to appear near River’s house, where she will reward you with the Boon for helping her. This is a buff that gives you +1 attack and revives you upon death, and you can get one in exchange for a rare seed if you ever need more. If you don’t have enough money, play Illy’s game in the Crocasino, her game is incredibly simple to win with this solution.

Prerequisites: Clear Tidal Mines

After completing the Tidal Miles, head back to the graveyard and take the staircase near your parents’ graves to find a thin section of water. Use the Lariat to move the nearby wooden barrels into the water to create a path to the ore.

Heart Ore #6
Location: Vann’s Point
Prerequisites: Clear Tidal Mines

Take the stairs near River’s fishing pond and use the Lariat to cross the river to reach another ore.

Coin of Crowl #3
Location: Dry Fountain
Prerequisites: Rust Knuckle

In the room with the boss door, knock a hole in the northeastern wall for a secret room containing a coin.

Heart Ore #7
Location: Dry Fountain
Prerequisites: Open the Waterway

After opening the waterways in the dungeon, backtrack to the room where you obtained the Rust Knuckle and head north a room from there. With the water now flooding the pit in this room, use the Rust Knuckle to clear the path and move the barrels into the water to create a path to the heart ore.

Location: Dry Fountain
Prerequisites: Open the Waterway

Head east as far as you can from the Rust Knuckle room and knock the metal barrels into the water to create a path to the southern end of the room, where an entrance into another room with a heart ore is hiding.

Heart Ore #9
Location: Boneyard
Prerequisites: Clear Dry Fountain

After completing the Dry Fountain, head back into the Boneyard and take the east exit from room 2 to find a room full of red boulders that you can now move. Follow this path to the very end where a heart ore resides as your reward for remembering an obstacle early on. While in this area, make sure to grab the chest in this wing for the Cursed Bones. We’ll use those soon.

Back outside the Abandoned Mine is a heart ore directly above the entrance blocked by a rock that you can now remove with the Rust Knuckle

Coin of Crowl #4
Location: Abandoned Mine
Prerequisites: Clear Dry Fountain

Head back into the Abandoned Mine and make your way to the chest where you obtained your Pickaxe. Going south from said chest will lead to a path that you can now cross with all 3 items you’ve obtained from the prior dungeons, with a coin at the very end.

Upgrade #3: Blessed Pickaxe
Location: Vann’s Point
Prerequisites: Cursed Bones

Bring the Cursed Bones to Father Xavier in the church. He’ll ask you to call Grant to start digging a grave for it. Sleep and then talk to Xavier again to discover the Cursed Grave. Clear the Cursed Grave to retrieve the Cursed Pickaxe item, which can then be brought to Father Xavier again and have him bless it to unlock the Blessed Pickaxe, adding a +1 bonus to your attack.

Upgrade #4: Cleated Boots
Location: Vann’s Point
Prerequisites: Blessed Pickaxe

After obtaining the Blessed Pickaxe from Father Xavier, head back to Grant and talk to him to have him give you a series of questions to determine how much he can trust you. If you answer in the order of “No, Yes, No, Yes”, he will trust you enough to ask for your help infiltrating a bandit camp. Follow through with this infiltration all the way through and you will be rewarded with the Cleated Boots upon return to Vann’s Point, equippable via Tess’ shop.

Talk to Pa for a few days in a row to unlock the prompt to pay back the money you stole from your parents prior to the start of the game.

Use the Rust Knuckle to climb the mountain east of Castle Vann. Near the peak is the entrance to the Magma Heart. Nearby said entrance is a staircase with a heart ore at the top.

In the room with the boss door, there is a puzzle with an iron ball and some moveable blocks. Solve the puzzle by getting the ball to the northeast corner of the room to Lariat over to it and reach the heart ore.

After defeating the boss of Magma Heart, you will be awarded the Fury Heart, the first of five colors you’ll be collecting on your journey.

In the big exterior area where you have to knock around iron balls to progress, move one of the iron balls to the northeast island and Lariat over to it to get the heart ore.

Upgrade #5: Anchor Greaves

This is another fetch quest very similar to the one for the Winged Boots, only this time every time you give an item to an NPC, they give you an item that someone else wants. In order to do this, bring the coffee from the dock warehouse to Tess. From there, you’ll be visiting Vulhara, Reskel, Mynir, Orima, and Wren in the void, Quinlan in Vann’s Point, then back to the void for Leer, Burg, Crelan, Tedra, and finally Ulni. Once this long quest is over, you will be awarded the Anchor Greaves for your trouble.

Prerequisites: Return to Vann's Point

Our next quest is with a new visitor to Vann’s Point; Kir Hasa. You can first meet her by going into the Sheriff’s Station. From there, you have to talk to her a few times to make her trust you; once south of Crocasino, once in Red’s Inn, and once outside Crocasino. After this, head to the Sheriff’s Station to get news that she’s injured! Run to her tent to check on her, then go grab Lynn to work her magic and fix her right up. From there, she will ask you to do you a favor; head to Crocasino and get the Ora Moa that Crocodile stole from her people. Talking to Crocodile from here will grant you access to the Backrooms, the end of which has the Ora Moa that you can turn in to Kir Hasa. Talk to Crocodile a second time to get the Dusty Key for later too

Upgrade #6: Lucky Boots
Location: Crocasino Backrooms
Prerequisites: Access to the Backrooms

While in the Backrooms, there is a special item for you to grab. From the room with the second crystal lock, go west and grab the key in the secret room south of the chest. Use it to unlock the normal lock in the crystal lock room and go north until you climb a ladder to the next floor. Follow the path to the chest containing the Lucky Boots, but you’re not done yet. Knock away the wall next to the chest to reveal a secret room with a crystal key in it, and another room beyond. Use the crystal key to open the chest in the center for the Eerie Mask, which is needed later. If this is too hard for you, swap your boots out to the Anchor Greaves we just got to ignore spike damage (you can use Keaton’s ham to still let you run).

After completing the Crocasino Backrooms, talk to Tess a few times (sleeping as necessary to progress) to get the Bone Key from her to unlock the Haunted Hall dungeon, just to the left of the staircase to Castle Vann

In the room to the right from the warp statue room, solve the puzzle of the hidden walls to reach the chest at the end containing your penultimate coin.

Upgrade #7: Boots of Graile
Location: Vann’s Point
Prerequisites: Clear Haunted Hall, obtain all boots

After obtaining all boots and giving Tess back her hairpin, she will reward you with your father’s boots. These combine the abilities of the Winged Boots, Anchor Greaves, Cleated Boots, and Lucky Boots all in one item, so have fun.

As you progress through the eastern wing of the dungeon, you’ll find a room with two black hole pits that can be filled up with metal barrels. Block the southern one to access that room and move the blocks inside to reach the heart ore.

In the room just past the boss door, knock a hole in the wall above the top left statue to reveal a staircase leading down. Dodge the cannons firing at you here and grab the chest in the center for one more shiny trinket

Color #2: Shattered Soul

Prerequisites: Clear Castle Vann

After defeating the boss of Castle Vann, you will be awarded the Shattered Soul, the second of five colors collected.

Location: Time Out I
Prerequisites: Complete Magma Heart & Castle Vann

Found in the northern end of the dungeon, in the room with many disconnected islands. Knock the iron ball in this room onto the northwestern island and Lariat over to it to reach the ore.

Prerequisites: Complete Magma Heart & Castle Vann

Found in the very western end of the dungeon, in a room full of moveable blocks. Move them in such a way that you can reach the top left segment to grab the ore.

In the room with the ladder leading down to the bottom floor, you’ll see an arrow in the floor pointing towards the left wall. Smack the wall where it’s pointing to reveal a golden secret!
Part 3
Coin of Crowl #12
Location: The Lighthouse
Prerequisites: Clear Time Out I

On your way through the lighthouse to confront Bolivar, open the chest in the second room full of bookshelves for one more coin.

Sidequest: Ashwood’s Dinner Party
Location: Vann’s Point
Prerequisites: Defeat Bolivar

If you haven’t already, go to the sheriff’s station to see a scene between Hugh and Siska (you may have to do it twice if Kir Hasa is there first), then head to Red’s Inn to see a scene of Siska smashing the wall. After that, talk to Hugh twice in his normal spots to get an invitation to Ashwood’s party, where a murder will happen. In order to solve it, the only clues you need are the left bed in the crew quarters, the journal next to said bed, and the kitchen fireplace. Accuse the Butler and rat him out to Hugh to end the party.

Prerequisites: Complete Ashwood’s Dinner Party

The next day after Ashwood’s party, go back to his mansion to speak with him to learn that he’s grown closer to Lynn! We need to nurture that! Head to the church for another scene, then back to the mansion to speak to Ashwood again. Sleep and repeat to get him to confess he loves Lynn. Go to her house to get a scene of Quinlan denying his blessing, then back to the mansion for one more scene. Sleep and talk to Ashwood twice in a row to see his determination to keep trying. Enter Lynn’s house one final time to see the wedding finally go off without a hitch! If you married Lynn before the party, this quest will be inaccessible and the feat with it.

After finishing Ashwood’s party, going back to Red’s Inn will see Hugh get abducted, which will open up Siska’s Workshop for us.

Upgrade #8: Magnet Ring

Location: Siska’s Workshop
Prerequisites: Witness Hugh get kidnapped by Siska

Just past the first crystal lock, rather than going north to the staircase that’ll lead to the boss door room, head south to find a path with a Crystal Key challenge, with the Magnet Ring as your reward for completion

In the room with the boss door, head to the right of the door and navigate the horde of Mecha Vanns heading north until you reach the chest with your final coin.

Upgrade #9: Bolt Anchor
Location: Vann’s Point
Prerequisites: All 13 Coins of Crowl, Anchor Greaves

After collecting all 13 coins and the Anchor Greaves (or Boots of Graile), head down to the abandoned beach house and dive into the water there to find a small room with an eyeball door. They request only one thing; the collection of all 13 Coins of Crowl. Show them that you have them all and you will be granted access into the room with the Bolt Anchor, an upgrade for your pickaxe that not only makes it stronger, but also shoots lightning at random enemies when you spin attack.

Sidequest: Hugh’s Wingman
Location: Vann’s Point
Prerequisites: Rescue Hugh from Siska’s Workshop

After Hugh is saved, he’ll have a big decision to make. To get to this choice, you’ll need to trigger a few conversations at the Sheriff's Station, one with Hugh in one of his spots, one in the Church, one back in the Sheriff’s Station, and a final one back at the church (sleeping between as necessary). A few scenes won’t trigger if you married either Siska or Caroline, but that won’t stop you. After seeing all these scenes, talk to Hugh to get him to ask you if he should marry Caroline. Saying yes will have him marry her, saying no will have him marry Siska. If you’ve married one already, he will marry the other.

Upgrade #10: Empowered Hand
Location: Forgotten Crypt
Prerequisites: Access to Crocasino Backrooms

During your mission earlier to infiltrate the Crocasino Backrooms, you were also given a key by Crocodile. Taking this key to the locked door behind the boss door in Castle Vann to unlock an entirely separate dungeon. Clearing this and defeating the boss will grant you access to a chained up rock. Smashing the chain will drop the Eye of Veritus, which will merge with your Dread Hand to become the Empowered Hand.

Upgrade #11: Flare Knuckle
Location: Crystal Caves
Prerequisites: Witness Hugh get kidnapped at the docks after being married

After Hugh gets grabbed by Revulan, speak to Roanoke at the hidden dock to be tasked with finding a map to her base so you can chase them down. To do this, head for the Crystal Caves, behind a cracked wall on the mountain. Navigating the southeastern wing will lead you back to the main room on the right side, where you can take the door directly right of the Frozen Crystal to find a red chest, only accessible when the dungeon is thawed. Opening this chest will upgrade your Rust Knuckle to the Flare Knuckle, doing more damage and allowing you to cross one block gaps in the ground.

Upgrade #12: Golem Ring
Location: Crystal Caves
Prerequisites: Witness Hugh get kidnapped at the docks after being married

After grabbing the Flare Knuckle, freeze the dungeon again and navigate all the way to the far northeastern corner of the dungeon to find a room with two chests, one of which contains the Golem Ring which can be equipped at Diana’s Ring Shop. While here, finish clearing this wing to collect the map you were sent here for by solving the puzzle beyond the locked pillar with the dungeon frozen, then thaw it to get through the waterfall

Color #3: Frozen Heart
Location: Crystal Caves
Prerequisites: Witness Hugh get kidnapped at the docks after being married

Head back into the Crystal Caves after collecting the map and complete the west wing of the dungeon and give up all your coins to the eye door to collect the Frozen Heart, the third of five colors for Colorgrave.

Upgrade #13: Spiked Ring
Location: Pirate's Pier
Prerequisites: Stindle’s Map

Head back to the secret dock in Vann’s Point and give Roanoke the map to be taken to the Pirate's Pier, where Revulan resides. Upon arriving, immediately get yourself caught and obtain the Cell Key from Caroline, then head north a room for a key. Use the nearby warp statue to get back to the start and head east until you reach the far eastern end of the docks. Use your key to go north from here to reach a chest with the Spiked Ring. To get back from here, either get caught again or head north from the room with all the lariat posts

Sidequest: Small Town Hero
Location: Pirate's Pier
Prerequisites: Stindle’s Map

After grabbing the Spiked Ring, backtrack to the docks and this time head into the western edge of the hideout, clearing this path to the boss at the end holding Hugh hostage. This will finally rescue Hugh and bring him back to Vann’s Point

Upgrade #14: Ring of Kings
Location: Pirate's Pier
Prerequisites: Complete Small Town Hero

After rescuing Hugh, return to Pirate's Pier and head into the main hideout to clear the eastern wing all the way through (including the worst puzzle in the game) to find the Ring of Kings at the end.

From this point, buy the Climbing Gear from Hackett and use it to reach the Waterfall Cave by climbing down the mountain. Complete this minidungeon to get the Bandana, which will be useful soon. You can also use this dungeon to farm the drops from Grelins; the Holy Relic, the Silver Mirror, the Wedding Ring, and the Painting, which will also be useful soon.
Part 4
Upgrade #15: Weapon Chain
Location: Vann’s Point
Prerequisites: Climbing Gear, Dad’s Harmonica

After getting the Climbing Gear and the harmonica from Pa’s room, head back into the Boneyard and take the path to the right of the boss door. You can now access the room across the pit with the Lariat, but the only thing in here is a warp statue and a massive hole. Use the Climbing Gear to get down safely and discover the Reflective Crypt. Complete this dungeon all the way through to have the opportunity to learn a song on the harmonica at the very end. With that, head back to your parents house on the mountain and play the song for the music box to unlock the Weapon Chain, giving your spin attack absolutely insane range

Color #4: Splinter
Location: Vann’s Point
Prerequisites: Bandana

Use the Bandana to reach the Mining Minigame just south of Magma Heart and give it a try. Your goal here is to climb down 6 levels by breaking rocks with varying hp levels to find ladders, eventually finding the Splinter at the bottom. With everything we have unlocked, however, this is incredibly easy. The highest HP a rock has is 5 which is a oneshot with our Bolt Anchor, Boots of Graile, and Boon, and the Weapon Chain can clear out a ton of rocks in very quick order. Reach floor 6 to collect the Splinter, our 4th of 5 colors for Colorgrave

Color #5: Sunset Paining
Location: Vann’s Point
Prerequisites: All 4 Grelin drops

After grabbing all of the unique items from the Grelins, you need to return them to their rightful owners in Vann’s Point. The Holy Relic goes to Father Xavier, the Silver Mirror to Ashwood, the Wedding Ring to Quinlan, and the Painting to Lynn. Once all these have been returned, talk to Lynn again and she will give you the Sunset Painting as thanks, the 5th and final color for Colorgrave!

Upgrade #16: Illusion Ring
Location: Time Out I
Prerequisites: Flare Knuckle

Backtrack to the room between the arrow floor room and the heart ore to the west and use the Flare Knuckle to reach the unique looking tile in the top left of the room. Interact with it to reveal a hidden chest with the Illusion Ring within.

Blessing #1: Light Blessing
Location: Vann’s Point
Prerequisites: All Five Colors

After collecting all five color items, bring them to Colorgrave and she will grant you access to Time Out II, the right side of her temple. Clearing this and then sleeping in your bed will grant you the Light Blessing, the first of five for our final challenge.

Blessing #2: Life Blessing
Location: Vann’s Point
Prerequisites: Eerie Mask

Give the blue hooded figure outside the Boneyard the Eerie Mask to start a chain where you need to find her in several places around Vann’s Point in a specific order. The order is; your parents house, outside the Lighthouse, outside Waterfall Cave, outside Boneyard, and back to your parents house. This final chat will warp you into an unknown realm where you will have to fight her. Successfully beat her for the Life Blessing, our second of five. If she’s not where she should be based on the list, just enter and exit a building to reload the map

Blessing #3: Water Blessing
Location: Tidal Mines
Prerequisites: Empowered Hand

With the Empowered Hand in your collection, a new path will open to the bottom left in the first room of the Tidal Mines. Navigate this new path to the end, where the Water Blessing awaits

Blessing #4: Earth Blessing
Location: Magma Heart
Prerequisites: Empowered Hand

From the room with the second fire puzzle, head south past the long bridge to see a new path that is open with the Empowered Hand. Navigate it to reach the Earth Blessing, our penultimate blessing

Blessing #5: Storm Blessing
Location: Pirate's Pier
Prerequisites: Rescue Hugh, Ring of Kings, 2 Blessings

Equip the Ring of Kings from Diana’s shop and make your way back to the Pirate's Pier. Head to the center of the dungeon, between the two wings we cleared previously, to find Revulan’s throne room. Talking to her with the Ring of Kings equipped and at least 2 blessings in our possession will cause her to hand over the Storm Blessing, the final one we’re looking for.

Taking the 5 blessings to the shiny rock outside the Boneyard and placing all 5 in their pedestals then examining the crystal in the center will complete the postgame story.

Upgrade #17: Daemon’s Ring
Location: Daemon’s Dive
Prerequisites: Complete the Postgame, obtain the Daemon’s Key from the crystal chest in Haunted Hall

Once everything else in the game is completed, you have one more dungeon to act as your final test; the Daemon’s Dive. A 9 floor dungeon comprised of harder versions of dungeons seen throughout the game, culminating in a harder version of that dungeon’s boss. Climbing your way to the very top and defeating the final boss will award you with your final upgrade.
