I couldn't find any information about this achievement in the guides, and my search led me to one guide outside of Steam, which I want to share here for your convenience. (I'll also leave a link to the original)

Glimpse of Disidian

Those two items need to be obtained before beating the boss in the Seat of Infinity as the area will shift/change after beating him and those items will be inaccessible.

CONSUMABLE ITEMS

#1 Baguette (1/2)
Location: Fallgrim Area – Beginning Place:
Found inside chest, there are only 2 baguette in all game, first 1 is found here in Fallgrim, other found in Seat Of Infinity – Shifting Archives, and we need (2) to max out familiarity, and you can do it, in single playthrough.

#2 Baguette (2/2)
Location: Seat Of Infinity – Dim Gate:
Found inside chest, on top floor hidden area, we need to use teleportation tower to get there, and we need (2) to max out familiarity.

#3 Roasted Rat
Location: Fallgirm Tower:
You can purchase it, from Vlas for 100 (tar), and also enemy drop it in Fallgrim area, you need (8) to max out familiarity.

#4 Boiled Frog
Location: Fallgirm Tower – Fallgrim Area:
Found in Fallgrim area, in a brigands camp, found inside pot, also some enemy drop it in all region, and in Shrine Of Ash, enemy drop it, and you need (4) to max out familiarity.

#5 Bronze Bell
Location: Fallgirm Tower:
You can purchase it, from Vlas for 1000 (tar), also found in chest, all region and found on ground sparkling item, in all game, you need (2) to max out familiarity.

#6 Golden Bell
Location: Fallgirm Tower:
You can purchase it, from Vlas for 1000 (tar), also found in chest, and found on ground sparkling item in all game region, you need (2) to max out familiarity.

#7 Inferior Moonshine
Location: Fallgirm Area:
You can purchase it, from Vlas for 150 (tar), also enemy drop it, in fallgrim area, and found inside chest and found in all game region, you need (8) to max out familiarity.

#8 Superior Moonshine
Location: Fallgrim Area – Beginning Place:
Found inside chest, in Fallgrim, also enemy drop it, in fallgrim area, and we need (3) to max out familiarity.

#9 Tarspore
Location: Fallgirm Tower – Fallgrim Area:
Found on ground sparkling item, in all game region, and you need (2) to max out familiarity.

#10 Weltcap
Location: Fallgirm Tower – Fallgrim Area:
Found on ground sparkling item, in all game region, and you need (8) to max out familiarity.

#11 Simple Lute
Location: Fallgirm Tower:
You can purchase it, from Vlas for 600 (tar), also found in chest, and some enemy drop it, in fallgrim, and you need to use it (10) time to max out familiarity.

#12 Scripture Of Euphoria
Location: Fallgirm Tower:
You can purchase it, from Vlas for 500 (tar), also found in chest in all game, and some enemy drop it, in Seat Of Infinity, and you need (3) to max out familiarity.

#13 Scripture Of Repent
Location: Shrine Of Ash:
Found on ground sparkling item, also enemy drop it, in Shrine Of Ash, and, also in Seat Of Infinity, found on ground sparkling item and chest, and you need (3) to max out familiarity.

#14 Scripture Of Despair (1/3)
Location: Shrine Of Ash – Monuments Of Ash:
Found on top barrel, there are only 3 i found, in all game, no enemy drop it, for me so far, and you need (3) to max out familiarity.

#15 Scripture Of Despair (2/3)
Location: Seat Of Infinity – Eternal Narthyx:
Found on ground sparkling item, there are only 3 i found in, all game, no enemy drop it, for me so far, and you need (3) to max out familiarity.

#16 Scripture Of Despair (3/3)
Location: Crypt Of Martyrs – Enshrined Sanctum:
After you defeated boss (Enslaved Grisha), found inside chest, in Enshrined Sanctum, there are only 3 i found, in all game, no enemy drop it, for me so far, and you need (3) to max out familiarity.

#17 Scripture Of Ardor
Location: Crypt Of Martyrs – Enshrined Sanctum:
After you defeated boss (Enslaved Grisha), found inside chest, in Enshrined Sanctum, where we got Scripture Of Despair, and also found on ground sparkling item, and all enemy drop it, in Crypt Of Martyrs, & Seat Of Infinity, and you need (3) to max out familiarity.

#18 Scripture Of Defiance
Location: Fallgrim Tower – Fallgrim:
Found on ground sparkling item (2), also found in chest, in Seat Of Infinity, and enemy drop it, in that area, and you need (3) to max out familiarity.

#19 Tainted Nektar
Location: Fallgirm Tower:
near to seat of infinity, you find corrupted sester, who sell it for 250 tar, also enemy drop it when fog is active, you need (2) to max out familiarity.

#20 Mortal Token
Can be purchased from Sester Genessa after fully upgrading one shell, also found in chest, and enemy drop it, all region and found on ground sparkling item, and you need (10) to max out familiarity.

#21 Spectral Token
Can be purchased from Sester Genessa after fully upgrading one shell, also found in chest, and enemy drop it, all region, and found on ground sparkling item, and you need (10) to max out familiarity.

#22 Ornate Token
Can be purchased from Sester Genessa after fully upgrading one shell, also found in chest, and enemy drop it, all region, and found on ground sparkling item, and you need (10) to max out familiarity.

#23 Untarnished Mask
Can be purchased from Sester Genessa after fully upgrading one shell, also found in chest, and found on ground sparkling item, some area, and you need (2) to max out familiarity.

#24 Etherial Diapason
Can be purchased from Sester Genessa after fully upgrading one shell, also found in chest, and found on ground sparkling item some area, and you need (2) to max out familiarity.

#25 Effigy Of Harros
Location: Fallgirm Tower:
Near to Seat Of Infinity, you find Corrupted Sester, who sell it for 500 (tar) unlimited stock, and also vlas sell it, and found on ground sparkling item, all game and some enemy drop it. all region and you need (4) to max out familiarity.

#26 Effigy Of Solomon
Location: Fallgirm:
Near to Seat Of Infinity, you find Corrupted Sester, who sell it for 1000 (tar) unlimited stock, and found on ground sparkling item, all game and some enemy drop it, all region, and you need (4) to max out familiarity.

#27 Effigy Of Tiel
Location: Fallgirm:
Near to Seat Of Infinity, you find Corrupted Sester, who sell it for 1000 (tar) unlimited stock, and found on ground sparkling item, all game and some enemy drop it, all region, and you need (4) to max out familiarity.

Location: Seat Of Infinity – Shifting Archives:
It’s only found in Seat Of Infinity, inside chest, and it’s (missable) if you don’t get first time, and you defeat final boss in this area, then environment & place will change, then you can get, only after new game+, so i recommend you get it first try when you come Seat Of Infinity, and you need (1) to max out familiarity.

#34 Slurry Of Tar (1/3)

Found on ground sparkling item, and only 3 you can find, in all game, and you need (3) to max out familiarity.

#35 Slurry of Tar (2/3)
Location: – Seat of Infinity – Shifting Archives:
Found on ground sparkling item, just near to final boss area, and you need (3) to max out familiarity.

#36 Slurry Of Tar (3/3)
Location: – Crypt Of Martyrs – Martyr’s altar:
Found on ground sparkling item, just near to final boss area, only after you defeated final boss in this area, and you going back with gland then you can see Slurry Of Tar, and you need (3) to max out familiarity.

#37 Unguent Tar (1/3)
Location: Fallgrim Tower – Fallgrim Area:
Found on ground sparkling item, and only 3 you can find, in all game, and you need (3) to max out familiarity.

#38 Unguent Tar (2/3)
Location: Shrine Of Ash – Monument Of Ash:
Found inside chest, and you need (3) to max out familiarity.

#39 Unguent Tar (3/3)
Location: Crypt Of Martyrs – Enshrined Sanctum:
After you defeated boss (Enslaved Grisha), found inside chest, in Enshrined Sanctum, and you need (3) to max out familiarity.

#40 Extracted Tar
Location: Fallgrim Tower – Fallgrim Area:
Found inside locked chest, fog has to be active, you must returned al least one main boss gland, and you have to give Thestus glimps, to active fog, then you can open locked chest, only on fog mode, also found in Shrine Of Ash, near to boss area inside chest, and you need (1) to max out familiarity.

#41 Nascent Tar
Location: Fallgrim Tower – Fallgrim Area:
Found inside chest, and also you can get it, as reward when you defeat all 3 final boss, in all region, and you need (1) to max out familiarity.

Can be obtained by finding all Etchings, are hidden to the wall and you have to attack that wall so they can show up, we already made guide where you can, find all 17 Etchings, also found on ground sparkling item, in all game and all region, you need (8) to max out familiarity,

#44 Glimpse Of Affection
Can be purchased from Sester Genessa after fully upgrading one shell, also found in chest, and found on ground sparkling item, all region, and, and you need (4) to max out familiarity.

#45 Glimpse Of Hope (1/2)
Location: Fallgrim Tower – Fallgrim Area:
Found inside locked chest, fog has to be active, you must returned al least one main boss gland, and you have to give Thestus glimps, to active fog, then you can open locked chest, only on fog mode, also found in shrine of ash, near to boss area inside chest, and same also in crypt of martyrs boss area,when you crawl in hole, inside you found chest, and you need (2) to max out familiarity.

#46 Glimpse Of Hope (2/2)
Location: Fallgrim Tower – Fallgrim Area:
Found inside locked chest, fog has to be active, you must returned al least one main boss gland, and you have to give Thestus glimps, to active fog, then you can open locked chest, only on fog mode, also found in shrine of ash, near to boss area inside chest, and same also in crypt of martyrs boss area,when you crawl in hole, inside you found chest, and you need (2) to max out familiarity.

Found inside locked chest, fog has to be active, you must returned al least one main boss gland, and you have to give Thestus glimps, to active fog, then you can open locked chest, only on fog mode, and you need (1) to max out familiarity.

#48 Glimpse Of Truth
Location: Fallgrim Area – Beginning Place:
Found on ground sparkling item, also found in chest, in all region in the game, and you need (1) to max out familiarity.

#49 Glimpse Of Disdain
Location: Seat Of Infinity – Dim Gate:
Found inside chest, on top floor hidden area, we need to use teleportation tower to get there, also in same area another 1 also can be found, where you see gold bowl on the pillar side on ground sparkling item, and you need (1) to max out familiarity.

#50 Glimpse Of Annihilation
Location: Shrine Of Ash – Sanctum Of Flame:
You need to defeat boss (Imrod, The Unrepentant), he will drop Glimpse Of Annihilation, after you defeated him, it’s already max out familiarity.

#51 Glimpse Of Insignificance
Location: Crypt Of Martyrs – Martyr’s Altar:
You need to defeat boss (Tarsus, The First Martyr), he will drop Glimpse Of Insignificance, after you defeated him, it’s already max out familiarity.

#52 Glimpse Of Oblivion
Location: Seat Of Infinity – Shifting Archives:
You need to defeat boss (Crucix, The Twiceborn), he will drop glimpse of oblivion, after you defeated him, it’s already max out familiarity.

#53 Corvid’s Glimpse Of Admiration
Location: Seat Of Infinity – Shifting Archives:
To get Corvid’s Glimpse Of Admiration you need to be in Horros shell, then speak to Brether Corvid, just use Effigy Of Harros, or change your shell in Fallgrim Tower, it’s already max out familiarity.

#54 Glimpse Of Cowardice
Location: Seat Of Infinity – Shifting Archives:
To get Glimpse Of Cowardice, you need to be in Eredrim shell, then speak to Brether Corvid, just use Effigy Of Eredrim, or change your shell in Fallgrim tower, it’s already max out familiarity.

#55 Thetus’s Glimpse Of Tenacity
Location: Fallgrim Tower:
Just speak with Thetus, in the Fallgrim Tower and give him, Glimpse Of Cowardice, which you got from Brether Corvid, and he will give Thetus’s glimpse of tenacity, already max out familiarity.

UNLIMITED USE

#56 Tarnished Mask
Given to you by Sester Genessa the next time you die after meeting her, any shrine she will give you, once you die there, and you need (1) to max out familiarity.

#57 Ornate Mask
Can be purchased from Sester Genessa after fully upgrading one shell, and you can only get it, from her, and you need (1) to max out familiarity.

#58 Tattered Vestment
Location: Fallgrim Tower:
Found on ground sparkling item, in second floor near to old prisoner, already max out familiarity.

#59 Disciple’s Rag
