This guide serves purpose to 100% Achievement running Mafia: The Old Country. This game is just a couple of days old, so bare with me.

These ones cannot be missed through the entire play through. Only way you can miss these if you just delete the game and never play again, but we will still be covering them.

Starting with:

Description: Enzo escaped the mine.
Chapter: Prologue - Tremori
How to Obtain: This is one of the very first achievements you'll earn. It unlocks after you successfully navigate and escape the perilous mine environment during the game's prologue or early first chapter, marking your first steps into the criminal underworld.



Description: Enzo found a new home.
Chapter: Chapter 1 - Famiglia
How to Obtain: After the events of the mine escape, Enzo will find himself in a new, perhaps unexpected, living situation. This achievement pops once you've settled into your new base of operations and advanced the story a bit further in Chapter 1.



Description: Enzo won the Palio.
Chapter: Chapter 2 - Palio
How to Obtain: In Chapter 2, you'll participate in the thrilling Palio, a traditional horse race. Simply win this race as part of the main story progression, and "Forza San Celeste" will be yours. This often requires you to master horse riding mechanics.



Description: Enzo learned the ropes.
Chapter: Chapter 3 - Pizzu
How to Obtain: As you delve deeper into the family's operations, Enzo will be taught the intricate ways of the Mafia. This achievement unlocks after a key story mission, likely in Chapter 4, where you're introduced to more fundamental aspects of the criminal enterprise.



Description: Enzo saved Isabella.
Chapter: Chapter 4 - II Barone
How to Obtain: This achievement is earned during a critical rescue mission, possibly in Chapter 4. You'll need to successfully locate and save Isabella from a dangerous situation, solidifying your bond and importance within the narrative.



Description: Enzo got made.
Chapter: Chapter 5 - Vendetta D'onore
How to Obtain: A pivotal moment in any Mafia story! This achievement signifies Enzo's official initiation into the family, confirming his loyalty and commitment. It unlocks after a significant ceremony or event where Enzo is formally "made" a member.



Description: Enzo freed the counterfeiter.
Chapter: Chapter 6 - La Difesa
How to Obtain: As part of a crucial operation for the family, you'll be tasked with rescuing a valuable asset – a counterfeiter. This achievement unlocks upon successfully freeing them from their captivity and securing their services.



Description: Enzo won the race.
Chapter: Chapter 7 - Spirito Sportivo
How to Obtain: While there's a specific achievement for beating the race in under 8 minutes ("Fastest Man in Sicily"), this achievement simply requires you to win a significant race later in the story, likely in Chapter 7, as part of a mission objective.



Description: Enzo and Cesare killed L'Ombra.
Chapter: Chapter 8 - Disgranzia
How to Obtain: This achievement is earned during a key assassination mission. You and Cesare will work together to eliminate a target known as "L'Ombra," who is likely a traitor or a significant enemy to the family.



Description: Enzo found the foreman.
Chapter: Chapter 9 - Industria
How to Obtain: As the story progresses, you'll undertake a mission to locate a specific individual, the foreman. This achievement unlocks once you successfully track down and confront them, revealing important plot details.



Description: Enzo killed his nemesis.
Chapter: Chapter 10 - La Festa
How to Obtain: A defining moment in Enzo's journey! This achievement unlocks after a climactic confrontation where you finally defeat a long-standing enemy or rival who has been a thorn in Enzo's side throughout the game.



Description: Enzo lost his mentor.
Chapter: Chapter 11 - Mattanza
How to Obtain: This is a somber but important achievement, signifying a significant loss for Enzo. It unlocks after a tragic event where a key mentor figure in Enzo's life meets their demise, impacting his path forward.



Description: Enzo exacted revenge.
Chapter: Chapter 12 - La Forza Del Destino
How to Obtain: Following a major setback or betrayal, Enzo will seek retribution. This achievement is awarded after a mission where you successfully carry out an act of revenge, fulfilling a crucial objective for the family or for Enzo personally.



Description: Enzo killed the Spadaro holdouts.
Chapter: Chapter 13 - Capiregime
How to Obtain: Towards the game's climax, you'll need to eliminate the remaining resistance from a rival faction or group, the "Spadaro holdouts." This achievement unlocks once you've successfully cleared them out, ensuring the family's dominance.



Description: Enzo and Isabella's story ended.
Chapter: Chapter 14 - La Merica
How to Obtain: This is the final story achievement, unlocking upon the completion of the main campaign. It signifies the conclusion of Enzo and Isabella's intertwining narrative, bringing their journey to a close.

Alright up next is your weapon achievements. Obtaining these is fairly easy as long as you switch your guns around and use them to eliminate enemies. The devs over at 2K worked tirelessly to provide us another Mafia game after five years, use everything they provide you.

Since these one's are technically a given, we will still be covering them.

Jumping into with:

Description: Killed 20 enemies with a Praecisione.
How to Obtain: Equip a Praecisione as your primary, use it to eliminate 20 enemies to unlock this achievement.



Description: Killed 20 enemies with a Lupara.
How to Obtain: Equip the Lupara as your primary, use it to eliminate 20 enemies to unlock this achievement.



Description: Killed 20 enemies with a Pump-Action Shotgun.
How to Obtain: Once again another shotgun, act like you're slaying demons in hell like DOOM 64 and eliminate 20 enemies to unlock this achievement.



Description: Killed 20 enemies with a Vendetti.
How to Obtain: Equip the Vendetti as your primary, use it to eliminate 20 enemies to unlock this achievement.



Description: Killed 20 enemies with a Modello C96.
How to Obtain: Like my farther on a drunken night when I was a child... Equip the Modello as your primary, use it to eliminate 20 enemies to unlock this achievement.



Description: Killed 20 enemies with a Repeater.
How to Obtain: Equip the Repeater as your primary, use it to eliminate 20 enemies to unlock this achievement.



Description: Eliminate 5 enemies with Knife Takedowns.
How to Obtain: Use every knife type and perform five knife special actions aka takedowns. Metal Gear Solid that ♥♥♥♥ and be a silent killer.

This section will be covering all of the miscellaneous achievements such as driving, stealth, and so on.

There is a ton of things you need to do, but luckily the game is open to move around in to get these done. Follow along and you should be just fine.

Starting off:


Description: Jumped a car over 40 meters.
Chapter: After Chapter 7 - Spirito Sportivo
How to Obtain: Find a fast car (the Carozella Mark 2 is a good choice after Chapter 7) and a suitable ramp, like the one near Pasquale's Gas Station that leads to the train tracks. Hit it with speed in free-roam mode to launch yourself over 40 meters.



Description: Drove at 120 km/h for at least 15 seconds.
Chapter: Best done in Free Roam after Chapter 7 - Spirito Sportivo
How to Obtain: Find a long, straight stretch of road, preferably outside of busy towns, and go as fast as you can. Hold it for a continuous 15 seconds, and this achievement is yours.



Description: Beat the train.
Chapter: Chapter 7 - Spirito Sportivo
How to Obtain: You'll face this challenge during a main objective in Chapter 7, "Spirito Sportivo." You're test-driving a racecar and need to outpace a train to the finish line. Just keep accelerating and avoid crashes, restarting checkpoints if needed.



Description: Completed the race in under 8 minutes.
Chapter: Chapter 7 - Spirito Sportivo ( Chapter Specific )
How to Obtain: During the race in Chapter 7, "Spirito Sportivo," you'll need to reach the finish line in less than eight minutes. This is often achieved naturally if you win the race.



Description: Eliminated 10 cars while escaping the Tonnara.
Chapter: Chapter 11 - Mattanza
How to Obtain: In Chapter 11, "Mattanza," during the "Defend the truck" objective, you'll be on the back of a truck being chased by enemies. Shoot and destroy 10 of their cars. This is generally unmissable as you need to destroy cars for the main objective anyway. Aim for the engine to make them explode.



Description: Customized a car.
Chapter: Preferred after Chapter 7 - Spirito Sportivo, but best done in Free-roam
How to Obtain: Head over to a garage and spend some Lira on making your ride look unique. Change the paint, add some accessories, any customization will do.

7. Riding in Style

Description: Customized a horse.
Chapter: After Chapter 4 - Il Barone
How to Obtain: Just like with cars, you can also personalize your horse. Visit a stable and apply any available customization option to your horse to unlock this achievement.



Description: Fixed a broken vehicle by using the crank.
Chapter: Best done in Free Roam after Chapter 3 - Pizzu
How to Obtain: If your vehicle breaks down while you're out and about, you'll have the option to fix it with a crank. Simply perform this repair action, and the achievement will pop.



Description: Bought all available cars from Pasquale.
Chapter: Best done after Chapter 7 - Spirito Sportivo or later toward end during Free Roam.
How to Obtain: This one requires a bit of cash! Save up your money and visit Pasquale's garage to purchase every car he has on offer.

Running on with the Miscellaneous Achievements Part 2


Description: Infiltrated the villa without being seen.
Chapter: Chapter 5 - Vendetta D'onore ( Chapter Specific )
How to Obtain: This challenging achievement requires a perfect stealth run during a specific mission where you need to infiltrate a villa. Take your time, observe enemy patrols, and use cover to remain completely undetected.



Description: Killed Mazzone without alerting his men.
Chapter: Chapter 13 - Capiregime ( Chapter Specific )
How to Obtain: During the mission where you confront Mazzone, you need to take him out without any of his surrounding men becoming aware of your presence or the deed.



Description: Defeated Don Torrisi without taking a hit.
Chapter: Chapter 14 - La Merica ( Chapter Specific )
How to Obtain: This requires precision and dodging! During your final confrontation with Don Torrisi in Chapter 14, you must defeat him in combat without sustaining any damage yourself. Channel your inner Elden ring and restart if you get hit.



Description: Reached the foreman's office without alerting enemies.
Chapter: Chapter 9 - Industria ( Chapter Specific )
How to Obtain: Similar to other stealth challenges, you'll need to navigate a specific area and reach the foreman's office without triggering any alarms or being detected by guards.



Description: Killed 10 enemies with explosives.
Chapter: I personally think after Chapter 4 - Il Barone during Free Roam
How to Obtain: Find grenades or other throwable explosives and use them to take out 10 enemies. Grouped enemies are your best targets here.



Description: Performed 30 headshots.
Chapter: After Chapter 4 - II Barone
How to Obtain: Aim for the head in every gunfight. You'll likely earn this naturally over time, but focusing on headshots will speed it up. Any weapon can contribute to this. Can be farmed by restarting checkpoints of combat.



Description: Killed 20 enemies with Knife Takedowns.
Chapter: After Chapter 3 - Pizzu
How to Obtain: For those who prefer a more personal touch, this achievement is all about stealth and precision. Get behind 20 enemies and perform knife takedowns. Note: Only one hit stealth takedowns count toward achievement!



Description: Cracked a safe without a combination.
Chapter: Best done in Chapter 3 - Pizzu ( MISSABLE ACHIEVEMENT )
How to Obtain: You'll encounter safes that require cracking without a pre-given combination. Successfully pick one of these to earn this achievement.

Description: Equipped a Charm
Chapter: End of Chapter 2 - Palio
How to Obtain: Equip a Charm onto your character.

I believe you can replay missions to get a different outcome, but this is more like a butterfly effect. Do one thing or another and you get a different outcome.

This section will be covering those.

Description: Killed Caccini before he reached Salt Flats.
Chapter: Chapter 13 - Capiregime ( Chapter Specific )
How to Obtain: During a specific mission involving Caccini, you need to be quick and decisive. Intercept and eliminate him before he manages to reach the designated Salt Flats area.



Description: Enzo refused Tino's order to kill in cold blood.
Chapter: Chapter 5 - Vendetta D'Onore or Chapter 13 - Capiregime
How to Obtain: This achievement involves a moral choice. During a mission where Tino orders Enzo to commit a cold-blooded killing, you must choose to refuse the order. This showcases Enzo's personal code. Also in Chapter 13, Tino will order you to shoot Costa and a cutscene will play after. If you take too long and not shoot, Cesare will be the one to kill Costa. Both chapters, mission will restart so you have a chance to play the mission out correctly.



Description: Met Isabella in secret.
Chapter: Chapter 3 - Pizzu ( Chapter Specific )
How to Obtain: This is an optional objective available early in Chapter 3, "Pizzu." At the very start of the chapter, while on your way to meet Luca at the Stables, you'll be given the option to "Meet Isabella in the garden." Follow the white marker to the garden, and the achievement will pop after the cutscene.

Description: Completed the story on Hard difficulty.
Chapter: Every Chapter.
How to Obtain: You need to play through the entire main story campaign from start to finish on the "Hard" difficulty setting. Prepare for tougher enemies and less forgiving combat.



Description: Completed the story on any difficulty
Chapter: Every Chapter.
How to Obtain: Play the game and finish it.

This mini section of the guide is to help those needing to go into "Free Roam" mode.

While also showing you how to go back to certain Missions you may have messed up in.

This section will help for collectible achievements or some of the Miscellaneous clean ups you might have!


Step 1: Reach Chapter 3 - Pizzu

Step 2: Head to the Title screen, from there click on Carcyclopedia, then select Explore, while even allowing you to replay missions.





While progressing through the story, will allow you the unlock more travel points around the map. I recommend completing the Campaign first, then going back over during your clean up for obtainable items. Makes things easier in my opinion!
