A lot of knowledge
1. Build your way up
Before heading straight into the hardest challenge the game has to offer, work your way up. I recommend beating most of the presets as they will help you get accustomed to the games new difficulty spikes. I recommend beating all preceding challenges before Suffer with the exception of Taste of hell, as the differences between taste of hell and suffer are miniscule.

suffer

2. Play minimized
After making this change and practicing a bit, I was able to beat suffer. Playing in minimized makes it so that the distance you have to cross with your mouse is much less. Which conserves stamina because you don't have to move your arm as much, and you can cross the distance in a faster time.

3. Be proactive + optimal customization settings
When in suffer, everything is turned up to the max, so you need to be on par with that. The monster will attack from every angle incredibly quickly, and you'll need to react in time. You need to respond near instantly to the monster's audio queues and pay attention to their eyes to see if they're red or not. I personally used the most recently released monster, but I believe all of them work. The best car lighting to differentiate whether or not the eyes are red or white in my opinion is the blue lighting. You can choose the one you like most but you should not pick the red lighting. It conflicts too much with the eyes and in my opinion can screw your runs up a lot.

4. Learn the deer's pattern
The mortal enemy of your gas meter in suffer is the deer. The deer hops from every tile every tick of the gamekid, and since the speed of the gamekid is maxed, you'll have to act very fast. If the deer starts on the right, you'll need to stay on the right, and the same for if the deer starts on the left. if the deer starts in the middle, you'll need to follow it. For example, if the deer starts in the middle and moves to the right, you should move to the right, or if the deer moves to the left, you should move to the left. The bunny occupies two spaces and is much easier to manage though is still more threatening than a basic rock and can easily cause you to run into it if you aren't paying enough attention to the gamekid.

5. What you should spend your attention on
The most pressing thing you should spend your attention on is dealing with is the monster. The monster serves as an immediate threat and you only have a few seconds before your run is over to deal with it. Next is the glitch, after around 5-10 seconds of the glitch being on your screen, your run will end. So often times you should find it after flashing the monster and click on it. Then finally is the gamekid, postponing your death until you've collected all the gas is important, but you also need to devote attention to collecting gas, inbetween clicking on the glitch and flashing the monster is when you should try to dodge the roads obstacles and be collecting gas.

6. Dealing with the glitch
The glitch has a filter that overlays itself on your screen a few seconds before it ends your run. At this point, you should maybe flash the monster once and then find and click the glitch. I find that the best opportunity to click on a glitch comes up when the monster overlaps with the glitch as it takes only 1 more second at most to click on the glitch after flashing the monster. I also recommend that in downtime after you've avoided obstacles in the gamekid and have fended off the monster, you locate the glitch. Your character can turn to the right and left very fast but turning behind you is a bit slower. So if the glitch is ever behind you its better to click it. If the glitch ever appears in front of you, you should stall it by not clicking it until the glitches effects start showing up. If it appears on the left or right, you can choose to click it immediately, or wait until it starts glitching and then turn to click it.

7. Finding the car key
The section of finding the car key is by far the most luck dependent, every other section is mostly skill based. Whatever side your car starts on is the side you want to go to, unless the key is in the middle of one of the 9 tiles, you want to stay on the edge of a tile relative to the middle of the map so you can quickly check the adjacent tile. For example, if your car started on the right side, you would go to the bottom right tile, then middle right tile along the left edge to be next to the middle tile, check the middle tile, go back to the middle right tile, then go to the top right tile, then middle top, then top left, middle left, and bottom left. If at any point you get the key you should head back to the car and take the fastest path to do so. With optimal pathing you should be able to make it back to the car with 1-2 bars of hunger left.
