I've recently made it to 35 and maxed my Ascension Points. Here are some things I learned.

Fishing
If you do any AFK activity and don't mind a simple macro then by far the best use of that time is fishing. You can start this from level 1 and just do it in your downtime between sessions. You can use AutoHotkey or your Logitech/Razer etc macro creation. It's the simplest macro; hotkey press (where the fishing pole will be), wait 10 seconds, repeat.

I recommend Azure City dock since this will provide all of what you will be wanting to collect.

You will get a lot of treasure map pieces, octorunners, and moongill. Hold on to the octorunners and moongill and store them in the bank for later. You will collect hundreds of these over time. Wait until level 30 to begin using the maps. I used the Best In Slot page on the Wiki as a guide (search for bis on the wiki). A large chunk of the recommended BiS gear come from the 30+ treasure maps.

Tip: Start your fishing with at least 1 of each corner of treasure maps, 1 octorunner, and 1 moongill to act as a place holder. You'll fish up junk items so you want to make sure to keep room for the important items.
Treasure Maps
Once you hit level 30 you want to start using the maps. Turn in your map pieces to Cecil Threbb in Faerie's Brake but make sure to keep at least 1 of each corner in your bank to be place holders in your inventory for your next fishing session.

You can turn in the map pieces in stacks without worrying about losing any. So if you have 56 top left, 48 top right, 42 bottom left, and 58 bottom left; take all of each (remember you're leaving at least 1) and drop them on Cecil Threbb to get 41 treasure maps. Bank the ones that are returned to you.

If you're just starting out with the treasure maps at level 30 and want to be safe then have your party "guard" nearby the treasure location but far enough away to not draw aggro. This allows you to run up, auto attack the chest, then get back before the wave of mobs spawn. This will allow you to safely pull the mobs a couple at a time. If you let them spawn on top of your party then your arcanist or other party member can be quickly killed.

The safest way to pull the mobs are gargoyles first, then skeletons, then spiders. Skeletons and spiders won't assist gargoyles, and spiders won't assist skeletons. If you attack in the other order then you can pull the entire wave at you. Gargoyles have the largest aggro range so sometimes you don't even need to pull with a spell. They'll just aggro as you approach. This will get easier and easier as you are leveling, getting lots of BiS gear, sivakrux's, and planar shards.

Use the best XP potions you can afford. The treasures will constantly provide level 1 xp pots but it's worth it if you can afford to buy higher tier potions. These mobs give much higher xp than other mobs at the same levels. With level 4 xp pots you will double the xp. There are 3-5 waves of 3-5 mobs that each will be giving between 300+ to 500+ xp per kill so it adds up quickly.
Blessing Items
As you're collecting better and better gear and loads of sivakrux's it's worth going into Fallen Braxonia from time to time to bless your items and the items of your party. I always used 1 sivakrux per attempt. I usually always came out ahead. Your party members have their own small collection of coins and shards so make sure to use those up first. After that you can hand them a coin then make the attempt. I didn't start going to purple blessing until I was much higher because the treasure maps will quickly get easy.

I recommend wearing journeying boots, having one of the party put on Spirit of the Steed aura, and the druid put on the Druid IV aura to get around quickly. The SimPlayers seem to acquire the Spirit of the Steed aura on their own so you may already have 1 or 2 that have it.
Octorunners and Moongill
The reason you've been saving these hundreds/thousands of octorunners and moongill is for the Demented Malaroth. You can buy Frowny Clownys and Azure Mead from Isaac Salaman near the Oddities merchants in Azure City. By this time you can probably roll through Prielian Cascade. Go to the oven in the small cabin on the second level of the zone and turn in stacks of octorunners, moongill, azure mead, and frowny clownys to get stacks of Odd Malaroth Feed.
Demented Malaroth
This can be tough at first but once you get it down then it will get easier and easier. You're probably well into making Ascension Points by this time. The safest method is to have your party "guard" a ways away. This allows you to walk up to the feeding bowls and sprint back in time for the tank to pull aggro rather than having it steamroll over you. You might want to have journeying boots on and/or have a Spirit of the Steed aura up depending on how tanky you are. Once you get more confident with this fight then it is definitely worth the tier 4 xp potions. You can slowly move your party closer and closer to the feed bowls. The last 20 AP I farmed was me strafing a couple steps left toward the bowls, spawning the Demented Malroth, strafe a couple steps to the right, kill it and repeat. You will be making 2304 xp every 20 seconds or so. You will quickly be able to get all 39 ascension points.

Update: I tested how many Demented Malroths I could kill on 60 minutes of an Elixir of Enlightenment IV. I was easily able to do 100 in 1 hour. That is 230,400 xp in 1 hour. That is over 6 Ascension Points per hour.
