Version 1.4 Patch
Jesus Christ be praised!

With the new 1.4 version patch there were added 3 new Codex entries.
To be more precise:
  • 1 new entry in the General category;
  • 1 new entry in the Society category;
  • 1 new entry in the Crafts category;

These seem to be connected with the Legacy of the Forge DLC and I'm currently still in Trosky region story-wise, so it'll take some time for me to start the DLC and get the entries.
Also it seems that at least one of the old Codex entries got fixed with this patch:

Codex: Fixed missing Smelters codex page.

These new entries were found and pointed out by CzechKronner, thank you!
- General category: Astronomical clock - unlocked automatically during the Main story of Legacy Of The Forge.
- Society category: Guild Life - unlocked automatically in the last Ragman's mission (Legacy Of The Forge - side content).
- Craft category: Papermaking - unlocked automatically in the last Ragman's mission (Legacy Of The Forge - side content).

Introduction
Before we start, I'd like to write a few sentences about how this guide came into being and why is it a Work in Progress. I've completed KCDII three times already with my 1st and 3rd playthroughs being as close to 100% as I could do. My 2nd run, however, was a speedrun of hardcore mode to simple get missing achievements.

The idea of fully unlocking the Codex came to me after beating the hardcore mode (and deleting its saves, heh). After my 1st playthrough I was missing 10 entries across all six Codex categories. Unfortunately, because I manually deleted my hardcore saves from in-game menu, I've no idea how many entries I've unlocked during it. So, I came to think that I'll just start my 3rd run of the game, doing all there is to do, reading everything etc. with the intention of finding these 10 missing entries.

Good news - I've found 7 of those entries. Bad news - I came short of several other entries, that I got unlocked during my 1st playthrough. Well, as the saying goes, one step forward, two steps backward.

Nevertheless, I've decided to go through with my notes that I was writing while playing and produce this guide. As you probably know, there's little to no information online on how to unlock Codex entries, however there's a KCDwiki page[kingdom-come-deliverance.fandom.com] dedicated to listing Codex entries, so far I've found a few incorrect pieces of information listed there but still it really comes in handy when you need to see which entries you're missing.

So, I want to thank people who've come across my questions about these entries on KCDII discussion page and I want to ask you to help me make this guide complete! If you remember how you've come across entries missing here let me know in comments down below! I'll check it and if your way of unlocking them works - I'll add it here. Thanks for reading it and I hope this guide will help you!

Cheers!
Important Notes
There are a few points which I'd like to clarify before we start:

1. There are several MISSABLE Codex entries, meaning that if you don't get them from doing or saying/asking something specific you won't get them. They will get [ * ] symbol before them, so you can use ctrl+F and then search for * to see them and hopefully not miss them. Keep in mind that doing all side quests and tasks, and exhausting all dialogues will prevent you from missing most of the missable entries. NOTE that there's a random encounter in Kuttenberg region which unlocks two Codex entries, but to unlock them you have to pass a hard Scholarship skill check. So make sure to level up your Scholarship, the best way to do it is as follows:
引用自 alleyezonme

  • Get your Alchemy to level 16.
  • Aquire Secret of Secrets perk.
  • Brew Henry's Fox potions[kingdom-come-deliverance.fandom.com] for increased experience gain.
  • Visit a Scribe or simply collect all books which you can find.
  • Put on your spectacles and find a comfortable spot to sit down.
  • While Fox's effect active read all the books that you have, but haven't read yet. Read both lore books (the ones where you can turn pages) and skill books (the ones where you spend time reading), learning crafting recipes and sketches also gains Scholarship experience. Exhausting some dialogues also levels up your Scholarship.

2. Throughout this guide you will see references to different aspects of the game, such as main quest, side quest, task, «name of» point of interest, perk in a certain skill tree etc.
I highly recommend PowerPyx's walkthrough[www.powerpyx.com] not only because it lists all main quests, side quests and tasks of the game, but also because you can see which of them are missable and, moreover, how to activate some of the not-so-obvious ones.

3. If you want you can use different interactive online maps to find points of interest [POI] mentioned in the guide. However, if you haven't already done so, I highly recommend you aquire the Explorer perk in the Scholarship skill tree. What it does is it removes all fog of war from the map and allows you to see all POIs there are, as well as fast travel points, hunting spots, merchants and every other thing that can be marked on the map. It's probably a bug, but I got this perk while still in Trosky region and when I got to Kuttenberg region I had all fast travel points unlocked and on top of that all Codex entries in Locations category which were tied to specific fast travel points in Kuttenberg region got unlocked automatically. My preferable way of leveling Scholarship can be found above in a quote.

4. It's also important to remember that there are a few Codex entries which have more than one way of unlocking them. The best example of it is the Burial entry in Society category. I've described the way I got this enry on my 3rd playthrough, BUT there's a way of getting it early (during my 1st playthrough I somehow got it early, but I don't remember exactly how or when, but again I'm 100% certain that it was much-much earlier than in Sedletz Monastery). The same goes for many entries that require you to visit some non-unique places for the first time. For example, if I wrote that you unlock an entry by visiting a carpenter in Troskowitz, it means that it's the earliest way of unlocking it, however there are other carpenters in Kuttenberg and in other places and thus there are other ways of unlocking that entry.

No. Educated Women is not linked to any "teach me" options. It's unlocked when you return the book to Rosa and then you're finishing the book with her. You need to pick Woman > Brave > Do you know any?

She then mentions several important women of history and the codex entry unlocks.

Well, it really seems to be the case.. I can't remember if I even got the option of asking Rosa to teach me something, apparently every time I've got that far the skill she teaches was maxed.

I believe the problem with Islam and Educated Women is that they are both tied to dialogue options for "teach me something". If you've already maxed out the skill(s) that the NPC can teach you, the dialogue options won't show up and you can't unlock the codex entries.

Metallurgy now unlocks when talking to Christian of Pisek, just before going after Buresh.
Tested on current game version (1.4).

So, I've found Johannes and pickpocketed the book.. Guess what? It gave me two entries:SirCatface: Wenceslas IV and Ius Regale Montanorum.

Way back someone told me that they've unlocked the Ius Regale by reading the books of the same name in Christian of Pisek's room during Via Argentum quest - that wasn't working for me. Now I guess with the 1.4 version patch the devs have fixed it and I guess reading the Ius Regale book during Via Argentum also unlocks the entry.

Yeah, not sure since I never got that codex entry on previous versions of the game (I didn't know what I should be looking for). But I double checked again and both the book you get from the upgrade of the forge and Johannes' own copy unlock the codex entry now.

About Ius Regale Montanorum, I have also previously pickpocketed Johannes for the first volume book but did not unlock the codex entry. Either 1.4 fixed something that was broken or there is more to it, perhaps the location is important?

Ha, actually found the same book - " Ius Regale Montanorum I" on a bookshelf inside Henry's room at his forge. I purchased the bookshelf upgrade and then it just happens to be there, along with bunch of other books.
