* Cosmetic mods should be no problem at all.
* Character mods are not supported at all currently, though that might change with the addition of letting people specify their own characters instead of just using the predefined one for a room.
* Item mods is a much trickier thing -- passive/active items you have on your local modded instance of the game will work, for you, however will not be represented with the appropriate image in the quick view HUD for others -- it will however show them an unknown item icon, so they can tell it's different. For custom pickups like hearts, pills, and cards, this can cause issues -- the shared healing will not work for the other players, and there is a potential for the shared effect from custom cards/pills to not trigger an effect (and due to how the modding environment works, there is a potential it crashes the game due to not having the right id on the other instances). There's much work to do in this area to make it better.
