THE NUMBER OF SYRINGES/AMMO IS THE SAME, but:
Syringes heal for less & enemies do more damage; and enemies have more HP (hence you need more ammo for the same quantity of enemies).
For reference: On easy, incoming damage ranges from 2% to about 10% per hit, and syringes heal for 100%. On nightmare, damage ranges from 15% to 60%, on 50% HP heal per syringe, and enemies having about 4 times the health (on Nightmare compared to Easy).
It's a meaningful difference because if you skip enemies, don't get surprised from ambushes from having played the game, etc. your hp/accuracy/ammo (not) wasted ought to be good enough to allow you to get by.
