Gravel quarry, 3 excavators: 100t stone produced -> 1.75t mixed waste
Iron mine: 20t iron ore produced -> 0.35t mixed waste (1.75t / 100t output)
I made the numbers on the sheet scale with output because 'per day' assumes that you are running the factory with 100% uptime and productivity, which usually isn't the case. Instead you can check the domestic production stats for a month/year, multiply them by the waste/100t and get figures for your actual production
I've added labels and two extra columns as per your sheet! From what I can see in your sheet, the numbers are not tying up because you are multiplying output (col F) by pWaste (col H). pWaste is the maximum daily garbage production stat from game, already expressed in tonnes. If you change it from "F5*H5*M5" to "H5*M5" then you should right numbers
The mistake is in here:

(also I beg of you, add the last few columns showing the actual t/day amounts, that's what we thirst about!)
While this is true, you can place a skid stand nearby and set one of the skids to hazardous waste to allow the gas power plant to keep its hazardous and non-hazardous waste separate.
Here are the results I got for testing the oil power plant's waste percentages. Your methodology for finding them is probably much better than mine, but I figured you may want to see them anyway:
