If you cannot unlock a specific achievement, it's likely that the achievement itself is broken. You can apply my custom patch to make it work if that's the case.

Serve every dish with 5-star score in Career Mode.

This achievement is not unlockable.

The game stores the perfect dish completion status in Steam user stats as multiple bit fields. Each bit field has to be linked to a stat named "A_DISHES_FIVE_STAR_XX", where XX = recipeID / 32. So there should be seven stats, which are from "A_DISHES_FIVE_STAR_31" to "A_DISHES_FIVE_STAR_37".

However, developers haven't registered these stats on Steam, so every request to update these stats will fail and skip any condition checks for achievement unlock, whichever recipe you served perfectly.

The description is wrong. This achievement can be unlocked in Sandbox Mode as well.

There are total 116 recipes from base content (ID 1022~1102, 1133~1135, 1140~1141, 1188~1190, 1192~1208) and 30 from Food Network content (ID 1103~1132).

Every recipe for ID <= 1132 and Roasted Christmas Ham (1140, 1141) has a corresponding 5-star achievement so you can easily track them down. Below is the list that doesn't have such achievement.

Fast learner.

Serve 3 dishes with 4+ star rating. This must be done in the tutorial, and unlocked when the tutorial ends.

Get a Golden Statue in all Competitions during one career.

There are 7 contests in the game. Each contest features a specific set of recipes, and you need some of these recipes unlocked in order for the contest to become accessible.

If all conditions are met, there is a 75% chance for the contest to occur.

Complete every daily quest in the game.

There are 29 quests in the game. Daily quests require at least one fame star for availability. Some daily quests have a fame star requirement, which means they are not available in early or late game.

You don't need to do all quests in a single run.


IMPORTANT:
For perfect taste/technical/temperature quests, you actually need "not perfect" score to complete the quest. Please follow "Custom Patch" link for more information.

Note.

• The quest completion status is managed via Unity PlayerPrefs. Go to HKCU\Software\Wastelands Interactive\CookingSimulator and find keys starting with "QuestDone_" to check your progression.
  
• The description of quest ID 121 is wrong. You have to serve the second dish within 60 seconds after you served the first one (not 2 minutes).

You missed a spot
Finish the game while leaving the kitchen clean after each day.

The description is clear but leaves ambiguity regarding whether the kitchen was left clean.

Extract the JSON from a save file and look for "PlayerMilestones". An entry named "DirtyKitchenAtEndDay" indicates a failure to maintain cleanliness in your space.


It's recommended to save every time before finishing a day and check if you "missed a spot" before proceeding to the next day.
