This guide is a continuation of the fantastic work - "Events Guide" by unnownrelic.
As far as I can tell, the author is no longer maintaining that guide, despite the fact that players continue to post new material in the comments section. For that reason, I am creating this guide, to continue the Great Work. I have completed the game many times, yet it is still my favorite game, a game I come back to over and over again, and eagerly await each update and DLC.
I will attempt to maintain this guide based on submissions HERE only.

PLEASE submit using a format that follows mine as much as possible, to make it easier for me to copy and paste into the guide.
At this point, I do not plan to use screenshots as it will reduce the work load, and they are not searchable any way.

Deep Damage - Mechanicus Depot
note: I recently used the option - 'Send Inquisitor Vakir to 'negotiate', and did not experience any kind of glitch as mentioned in the other guide. The result was:
Construction Suspension Cancelled ( and nothing else)
However, about 7 days later, it triggered the 'Deep Damage - Delayed Too Long' event.
Deep Damage - Delayed Too Long
"...failure to address structural malfunction..."
This even occurs about a week after "Deep Damage - Mechanicus Depot" if you choose the Inquisitor option.

Mediation Chamber Damaged
Exterminatus Damaged
Deep Wounds - Hospitaller Facility
Follow-up mission to Deep Wounds - Recovery Suspended.
  • We will wait until they arrive
    Result: Triggers 'Deep Wounds - Recovery Underway' event. Ship immobilized for 10 days, unable to cancel. After the 10 days, the knight is Wounded (and so must still recover) and gains Autosanguine Augmetics (+1 Armour, +2 HP).
  • Grant him the Emperor's Peace
    Result: One of your wounded knights dies.
  • See if they can be bribed
    Result: Research suspended (10 days)
    Libris-Malleus Damaged (Upgrades offline)

    Triggers 'Deep Wounds - Recovery Underway' event. Ship immobilized for 10 days, unable to cancel. After the 10 days, the knight is Wounded (and so must still recover) and gains Autosanguine Augmetics (+1 Armour, +2 HP).
    note: I experienced this April 2025. not sure if the result of this choice is random, or was previously bugged and was patched. This result makes more sense than Libris-Malleus Damaged IMO, but is also redundant with waiting.

Deep Wounds - Recovery Suspended
It is related to an Knight being critically injured.
  • Set coordinates to the facility.
    Result: Recovery Suspended on the wounded knight;
    Random planet gains new mission: Deep Wounds - Hospitaller Facility. There is a timer on this mission.
  • Grant him the Emperor's Peace.
    Result: One of your wounded knights dies.
  • Conduct risky medical procedures.
    Result: Knight Lost and Ectar - Indignant (30-45 days) -50% XP rate
    note: it is unclear what factors may influence the results. Based on one report.
    note: Like 'The Emperor's Peace - Battle Scars', results may improve the more upgrades you have for your Apothecarion, to include full recovery.

Dreadnought Malfunction - Fury of the Machine
(Happens at random, no triggers apparently) From the "Duty Eternal DLC"
  • Ignore it: (His rage might be useful)
    Result: +1 Melee Dmg, -2 WP
  • Send him away to expiate his bloodlust: (The Dreadnought will be away for some time)
    Result: Dreadnought away on a "mission" for 45 days, Dreadnought Level up when returning
  • Ask the adeptus Mechanicus to Help: (you will lose 2 Archeotech)
    Result: Lunete is wounded, lose some servitors
  • Assign a Tech-Marine to train self-control: (Requires a battle-ready TechMarine)
    Result: tech marine is gone for 30 - 60 days and when it comes back the dreadnought gets the Discipline Trait (+2 max willpower)

Failing to repair a damaged system, and waiting too long, triggered this event. In my case, it was Exterminatus that was damaged.

Grand master report, Day 691

He asks what else he can send via the nullship as it's getting harder to locate the Edict's signal.

Options are:
- Request more knight
- Free exterminatus torpedo
- Chapter standard (+50% XP buff, PERMANENT)

I chose the standard as I was still leveling some assassins.

Redrick - yw. I could not agree more. The dev's are 40k fans, and I find it hard to believe that they 'walked away.' I suspect Frontier, which bought the studio after this game was released, is the problem. I am still hopeful that they will do another game like this in the future.

Thanks very much for continuing this guide, it's a great resource.

It's a real shame the developers have walked away from game as it has the basis for an excellent ongoing Warhammer franchise. Imagine a campaign against Orks, Aeldari or another traitor legion (Khorne, Slaanesh etc.)!

Deep Wounds - Recovery Suspended

This event seems to share a coin-flip with Battle Scars - The Emperor's Peace. As a corollary to that, it seems relatively likely that this event's "risky medical procedures" shares the same set of outcomes and the same RNG table.

In "Unanticipated Storm - A Risky Course", allowing Vakir to guide you results in he being wounded for 20 days and Stratagems being locked.

In "Death Guard Fleet - Grappling tactics", firing on the Death Guard ship within the warp while possessing the enhanced Geller field prevents you from taking any damage. I got the screenshot if necessary.

Day 88 Grandmaster Report, has option to Assign Apothecary to prepare report. An Apothecary will be unavailable fro 45 days and on the day 208 Grandmaster report you will gain 2 requisition.
