Introduction
Here's an achievement guide for a game that is no longer even listed on Steam. Enjoy!

This guide is separated into sections based on which DLC's are required for certain achievements. If you do not own a particular DLC, you cannot get the achievements listed in that section.
Base game
All of the following can be achieved in the base Pandemic game with no need for DLC content.

Scholar: Complete the tutorial.
Exactly as it sounds: complete the optional tutorial.

Fighting Back: Treat your first disease.
This will pop the first time you perform the Treat action in a game.

Clinical Laboratory Success: Cure a disease.
Achieved the first time you Cure a disease in a game.

Effectiveness: Use the Discover a Cure action twice in a single turn.
Cure two diseases in the same turn. The easiest way to do this is on the Scientist's turn with help from the Researcher but other options are possible.

Flawless Victory: Win without triggering any outbreaks.
This is a bit luck-based as Epidemics can trigger immediate outbreaks but should happen eventually. Try this on the easiest difficulty if it's proving elusive. The Quarantine Specialist and the Contingency Planner (who can recycle One Quiet Night, Forecast, etc.) are also useful for this one.

On the Brink of Extinction: Win a game on the last round.
Win when there are zero cards remaining in the Player Deck i.e. the last turn possible in the game.

Eradication!: Eradicate your first disease.
Eradicate a disease for the first time i.e. cure it and then remove every remaining cube of that disease from the board.

Disease Hunter: Eradicate 2 diseases in a game.
As it sounds: eradicate two diseases in one game. This and the following eradication-based achievements are somewhat easier with the Medic.

Disease Exterminator: Eradicate 3 diseases in a game.

Disease Annihilator: Eradicate all 4 diseases in a game.

Easy but Not so Easy: Win a game on introductory difficulty.
This pops when you win your first game on the easiest difficulty or higher.

Getting Good: Win a game on standard difficulty.

The World Needs a Hero: Win a game on heroic difficulty.

Still Got Some Time: Win with 12 or more cards left in the player deck.
As it sounds: win with 12 or more cards remaining in the Player Deck. This and the following Player-Deck-related achievements are obviously simpler to achieve on easier difficulties.
Still Got Plenty of Time: Win with 18 or more cards left in the player deck.

Too Much Time on Your Hands: Win with 24 or more cards left in the player deck.

Starting Duo: Win at easy level or higher with the team: Researcher, Scientist.
Win a two-role game with the Researcher and the Scientist.

Dream Team: Win a game with the "dream team" (the medic, the scientist and the researcher).
Win a three-role game with the Medic, Scientist and Researcher.

Advanced Squad: Win at easy level or higher with the team: Contingency Pl., Dispatcher, Op. Expert, Quarantine Spec.
Win a four-role game with the Contingency Planner, the Dispatcher, the Operations Expert and the Quarantine Specialist.

Pandemic Lover: Win 5 games.

Pandemic Pro: Win 15 games.

Pandemic Addict: Win 40 games.

Pandemic God: Win 100 games.
On the Brink: Roles & Events
These achievements require the On the Brink - Roles & Events DLC. Achievements for this DLC use "difficult" to mean "Heroic" difficulty.

Again, because I keep getting questions and comments about this: DIFFICULT MEANS HEROIC.

Fabulous Five: Save the world with 5 players at legendary difficulty.
Five-role games are only possible with this DLC and Legendary difficulty is also only available with this DLC. Play a Legendary difficulty game with five roles and this achievement will pop upon a win.
Other versions of this game call this achievement Like the Fingers of a Hand or United we stand. Some versions of this game also reward I Am Legend for winning a game on legendary difficulty with any number of players.

Full of Knowledge: Win at difficult level or higher with the team: Researcher, Epidemiologist, Archivist.
Win a 'difficult' (Heroic) three-role game with the Archivist, the Epidemiologist and the Researcher. Again, "Difficult" is the same as "Heroic" difficulty.

Shock Therapy: Win at difficult level or higher with the team: Medic, Quarantine Spec., Containment Specialist.
Win a 'difficult' (Heroic) three-role game with the Containment Specialist, the Medic and the Quarantine Specialist.

Lab Tour: Win at difficult level or higher with the team: Scientist, Op. Expert, Field Op.
Win a 'difficult' (Heroic) three-role game with the Field Operative, the Operations Expert and the Scientist.

Without Borders: Win at difficult level or higher with the team: Dispatcher, Operation’s Expert, Troubleshooter.
Win a 'difficult' (Heroic) three-role game with the Dispatcher, the Operations Expert and the Troubleshooter.

Each to Their Own: Win at difficult level or higher with the team: Troub., Gen., Contingency Pl., Archivist, Field Op.
Win a 'difficult' (Heroic) five-role game with the Archivist, the Contingency Planner, the Field Operative, the Generalist and the Troubleshooter.

Team Work: Win at legendary difficulty with the team: Researcher, Dispat., Op. Expert, Contingency Pl., Epidem.
Win a five-role game at Legendary difficulty with the Contingency Planner, the Dispatcher, the Operations Expert, the Researcher and the Epidemiologist. The Field Operative is pictured in the achievement image instead of the Epidemiologist. Candidly, I can't remember which one is actually required for the achievement so do please correct me if I'm wrong.
On the Brink: Virulent Strain
These achievements require the On the Brink - Virulent Strain DLC.

Eradication: Virulent Strain: Eradicate the virulent strain for the first time.
Eradicate the disease that has become the virulent strain.

Immune Response: Save the world from a virulent strain at normal difficulty or higher.
Play and win a virulent strain game on at least 'normal' difficulty.
Other versions of this game give the achievement Up to the Challenge for beating a game with the virulent strain challenge active and Are You Not Entertained? for beating the virulent strain challenge 10 times (on Google Play this may be bugged and therefore impossible to get).
On the Brink: Mutation
These achievements require the On the Brink: Mutation DLC.

Clinical Laboratory Success: Mutation: Find a cure for the purple disease for the first time.
Cure the purple disease (hand in five city cards including at least one of a purple-infected city at a research station).

Nobel Prize: Save the world from a mutation while having discovered the 5 cures.
There's more than one way to win a mutation scenario and one is to find cures for all five diseases (the other is to cure the main four and then entirely remove the purple disease). Win a game this way for this achievement.

Eradication: Mutation: Eradicate the purple disease for the first time.
To be clear: this specifically means 'eradicate' i.e. cure and then remove the disease. Winning by simply not having the purple disease remaining on the board does not count.

Cool Headed: Save the world from a mutation at normal difficulty or higher.
Win a mutation scenario game on at least normal difficulty.
Multiple DLC
These achievements require several different DLC to obtain.

Experimental Medicine: Save the world from a virulent strain at legendary difficulty.
Legendary difficulty is part of the Roles & Events DLC, the Virulent Strain is from the eponymous DLC. Win a game as described for this achievement.

Sixth Sense: Save the world from a mutation at legendary difficulty.
Legendary difficulty is part of the Roles & Events DLC, the Mutation is from the eponymous DLC. Win for the achievement.

Flawless Victory: Win a game with the virulent strain and a mutation at legendary difficulty.
This requires every DLC: Roles & Events for legendary difficulty, the Virulent Strain and Mutation from their respective eponymous DLC's.

You can certainly try. I seem to remember getting them on a harder difficulty but I got all of these achievements and wrote this guide many years ago so who knows.

Have you tested those achievements specifically with 5 epidemics to confirm they won't work?

@Dark Nero As I say in the guide and in comments below, the On the Brink DLC uses "difficult" to mean "heroic,"

I've checked difficulty names in the release build, they are:
2 Very Easy
3 Easy
4 Normal
5 Hard
6 Heroic
7 Legendary

So I suppose "difficult" may refer to 5 epidemics.

@Ragenarok Also if your testing/searching unveils the answer for which roles are actually required for "Team Work" then do please let me know; thanks.

@Ragenarok I think that "difficult" for all of the achievements you name should mean "Heroic", as mentioned at the beginning of the "On the Brink: Roles & Events" subsection. I'll edit the guide to try and make this clearer.

I know I have a few of these achievements, but I unfortunately did them on Legendary so I can't narrow it down.
I'd like to play on the easiest possible eligible difficulty so I can actually play games out and enjoy this game for the board game port of the game I enjoy. and actually rack up wins for the cumulative achievements like Pandemic God.

I'll report back if I discover the answer elsewhere or through testing. But if someone knew this information they could save me having to apply the scientific method on this one.

Long version:

It's been a very long while since I played this. And for good reason I burned out on just re rolling games. A lot of getting many of these achievements is fishing for the ideal starting hands. Treating it like a game of slot machines.

Anyways. I came back to it to try and mop up some of these achievements and really beat this game. After all, from what i can tell, I've done some of the hardest stuff first.

However it appears that game has undergone quite a few changes since I played. From the DLC addition to the DLCs, down to it's terminology.

What is "difficult level or higher?" as referenced in "To each their own", "Without borders", "lab tour" , "shock therapy", "full of knowledge"?

TL;DR many achievements refer to a "difficult level or higher"

But the he difficulty settings to choose from are:
1 - Training
2 - Novice
3 - Introductory
4 - Standard
5 - Heroic
6 - Legendary

Did Difficult get renamed? which is it? are still these obtainable as of the latest version of the game??
