Click on New Game and select Hard in the bottom left corner.

Use the hotkeys 'S+T+L' (S+T first, then L) to skip levels. After using and automatically returning to menu, you may have to click continue and skip one more level.

After completing all levels, the Level Selector in main menu will be unlocked. Select the 3rd level and repeatedly run (SHIFT key) to the water to kill yourself.

After completing all levels, the Level Selector in main menu will be unlocked. Select the 43rd level and there will be two circles of enemies to kill below the first surface. The achievement for killing the first enemy is unlocked while playing while the others you need to go to main menu to pop.
