1. Check your monitor resolution is set exactly to the one you chose during modding.
2. Check if your monitor has any scaling enabled. It should be set to "100%" to match the resolution.

I can confirm that these instructions work, and I was able to get my game running in 3440 x 1440.
However, I did have the following issues:
First/
"Copy the two highlighted files and paste them into your KOTOR game directory."
This part of the guide seems to indicate that two files need to be pasted from the mod folder into the game folder, however it does not say which two files. It uses the word "highlighted" - but I could not make any sense of that. What is supposed to be highlighted? It almost seems as if there's supposed to be an accompanying screenshot to indicate the "highlighted" files but there was nothing of the sort.
Eventually I worked it out from reading the PDF that comes with the mod.
I feel this part of the guide needs to be written more clearly, it is very vague.
Second (and I guess this mostly comes down to personal preference) Launching the game in ultra-wide screen with all the UI mods installed makes it look great, however I suffered a noticeable drop in framerate. I could probably have put up with that if not for the other issue I experienced.
The UI, menus, dialogue options etc. are obviously much, much smaller, even if the mod puts them in the right location. It's harder to read the text, but again I could have coped with this if not for the map screen , which is now displayed as a tiny square, almost thumbnail sized.
This proved to be a deal breaker for me, given the importance of the map. I ended up reverting all the changes I'd made and just going back to the non-widescreen vanilla version.

One minor suggestion to OP, I don't see in this guide which files you're referring to in the "KOTOR High Resolution Menus 1.5" section, but I was able to get this from the video, so no worries.
