Someone made a python script to automate most of the process. It's much easier and supports custom steam library locations. Check it out here: https://psteamcommunity.yuanyoumao.com/sharedfiles/filedetails/?id=2909925037
Commands only work if your Steam library is in its default location.

Each step has only one command. Don't try to split them up by line, just copy the whole block.
Preparation
1. Download the latest release of the patcher (thcrap.zip):
https://github.com/thpatch/thcrap/releases

2. Extract thcrap.zip into your game folder
(This command only works if you downloaded it to the "Downloads" directory)
unzip $HOME/Downloads/thcrap.zip -d $HOME/.local/share/Steam/steamapps/common/th18/
Installing the Patch
1. Rename th18.exe to th18_jp.exe
mv $HOME/.local/share/Steam/steamapps/common/th18/th18.exe $HOME/.local/share/Steam/steamapps/common/th18/th18_jp.exe

2. Rename thcrap.exe to th18.exe
mv $HOME/.local/share/Steam/steamapps/common/th18/thcrap.exe $HOME/.local/share/Steam/steamapps/common/th18/th18.exe

3. Define the game's location (honestly just paste it in it works)
mkdir -p $HOME/.local/share/Steam/steamapps/common/th18/config && echo '{"'th18'"':'"'Z:$HOME/.local/share/Steam/steamapps/common/th18/th18_jp.exe'"}' > $HOME/.local/share/Steam/steamapps/common/th18/config/games.js
note: this looks very strange since steam doesn't allow backslashes in guides. still works though.

4. Run the game (not "Game Setting") through Steam

5. Go through the installer and select "English language pack" (the default)

6. On the next page you should see your game. just click next.

7. Click through the next screen and finish the installer

8. Rename th18.exe to thcrap.exe
mv $HOME/.local/share/Steam/steamapps/common/th18/th18.exe $HOME/.local/share/Steam/steamapps/common/th18/thcrap.exe

9. Rename thcrap_loader.exe to th18.exe
mv $HOME/.local/share/Steam/steamapps/common/th18/thcrap_loader.exe $HOME/.local/share/Steam/steamapps/common/th18/th18.exe

10. Add this to your game's Steam launch options
"en.js" th18

11. Run the game!!!

You do you of course, but I assure you the python script adds no significant overhead after the initial set up.
If thcrap is already set up all it does is edit the command line to run thcrap_launcher.exe with the right arguments and execs [en.wikipedia.org] into it, so it won't even leave an extra process behind :-)

Awesome!!! This was definitely much easier, especially for steam deck users on touchscreen. I'm not a huge fan of running a python script every time I launch the game, but that's just me. I'll be sure to link to your guide!

I just bought the game myself and decided to make a script to automate the process.

Thank you so much for this guide! It really helped. Has worked for me on 17.5 and 18.5 too. Better than the other method I used before by downloading the stand alone patches as those aren't available for all the games.
