There are 2 recopies that uses the same same ingredients. If you make the wrong - you will miss the achievement.

This require to rescue father Bezoir!!! If you kill or transform him, you will miss the achievement.
It is strange that, that is not mentioned anywhere I checked! It costs me couple of hours replay. Luckily, I had feeling that will affect end-dlc, so I made a save at that point.Otherwise I will have to replay the whole DLC.
Helping farmers is not enough to get a bison!!!
It seems that you need Animal Whisper 9 before "helping". Also other sources says that you need to talk to Knox's in the chapel (in the city) before you start the quest.
I can't confirm or deny which of the two is needed, because I failed in both, but for sure I missed a bison and thus the achievement :(
Icedfate already said about "Electronic Countermeasures" and that is very easy to miss!!!
One thing that is not actually missable, but might be quite misleading is "Plasma Spitter" in Tellurium Mine. If you kill all fast the turret will not be broken, so there is no "Plasma Spitter". In that case you will have to manually kill (destroy) the turret to grab the weapon.
but electronic countermeasures is missable as well, if the player chooses any outcome that loses rep with the gippers before finding the random encounter outside.
also missable is the railgun if you don't bring valor back alive
