Introduction
I created this short guide for those trying to unlock "The Gem in the Tower" Isle of Siptah DLC Achievement, as I was struggling with it myself.

After finding all locations shown in the screenshots, the achievement unlocked for me (tested in version 3.0.2).
Main Island

After following Live Map
If you used the Isle of Siptah live map and are struggling to unlock the achievement, make these checks. Finding all locations marked in this section should unlock the achievement.

1. On the bottom-left island, there is a place called Duskholme, which was not marked on the live map for me (C6).

2. North of Tower (center of map), make sure both locations are discovered (I10).

3. On the bottom-left island, there is a place called Cracked Vault (F4).
More difficult to find locations
1. Fuming Cave, little cave entrance with green smoke (D9)
2. The Depths, underwater cave. Need to swim close to or inside cave (C11)
3. Sweetwater Falls. Stand on little piece of land at the bottom of the waterfalls (F12)
4. The underclaw, entrance between the cliffs (E9)
5. Riverwatch Keep and The Lonely Grave, one near the gate and one on the hill (N10)
6. Glowing Cavern, entrance to underwater cave is north of shipwreck (G6)
7. The bloodmine, walking down to the bottom part should unlock it (F6-F7)
Age of Sorcery - Chapter 3
Following location also needs to be unlocked with Age of Sorcery - Chapter 3:

1. Cave of Kurak at N10. The entrance to this cave is next to Riverwatch Keep gate (also N10).

No need to find all this places! You can find only specific place\places to get achievement.

For some reason I'm unable to post a guide myself But if anyone is looking for a way to get the following achievements: The Road of Kings, O Sleeper Awake, The God in the bowl, What do i know of Cultured ways?

The achievements seem to be bugged and most likely because of the rework done to "Journey" missions.

No matter how many altars,wheels of pain, beds u make it will not matter. The achievements for me only triggered when I was completing the Journey missions with corresponding objective like: "place or access bed,altar,wheel of pain..."

And for the Road of Kings it triggered for me when i finished all of the Journeys, even though the achievement description said "finish first journey"
Another way to get it could be if you follow the steps of the old "chapter 1" missions.

I think they reworked the Journey but forgot to update the achievement triggers or something. It could also just be some bugs because I had a lot of trouble with the "O sleeper awake" achievement.

Don't forget to enter inside the Tower to find the Voidforge and complete the achievement.

Ich hatte alle Standorte musste aber zum erhalten des Erfolges "in den Turm" rein gehen und etwas in der Mitte um das Blaue Feuer Laufen. :winter2019neutralyul:

Using god mode or single player shouldn't disable the achievement

I've entered the tower and I have not been able to achieve it. It might be that I'm on my own single-player server and entered into god-mode to move around the map more efficiently. Has it been confirmed that one has to "walk" the entire-game to achieve this rather than being in god-mode?

Seems like there is something strange going on with the detection of the achievement since last update :/. From what I can see you have all locations, could you try to enter inside "the tower" location in the middle of the map? That trigger it for others

I've been trying to 100% complete the map but I think you're missing a location or two even after the update. Can anyone locate the place I'm missing on my map? I'll post a link to the reddit images I posted just now:
