Load up the map Foundry on Halo 3, can be on forge mode or custom games.

You will want to stand in this are here in the photo.

Look to the left and you will see a small pile of trash.

If you look closely in the trash you will see a small hippo toy. Look at it or shoot it and you will get the achievement!

@Elvick not always, ive got it on two accounts one i just had to glance at it and on the other i looked at it for a solid 30ish seconds and nothing happened until I shot it.
