Launch the game.

Beat all 12 levels. The developer created this guide which shows the solution for every level.

Die to the spikes five times.

These ones take a bit more time but really aren't that hard when you know how.

Reach the end of any combination of levels 50 times. The fastest way to get this one is to play through all 12 levels, beat levels 1 and 2 again, then play level 3 over and over again until you get the achievement (you can load any level you have cleared by pressing Escape and selecting it).

This is NOT total time played; you have to play for two full hours in a single play session. The easiest way to get this one is to launch the game, select graphics quality to fastest, select windowed mode, then mute the game, minimize it, and let it run in the background for at least two hours. The Steam overlay (Shift + Tab) will display the length of your current play session in the top-left corner for reference.
