Very last challenge took me 2 tried, 15:22 first time and 13:38 second time. Perfectly explained!
I have an addition: in "Worlds of Hurt" re-enrering a room respawns the NPCs, so photos can be made again without "subject already taken" penalty.

Picked up the last one (122) no achievement, counted everysingle one in the menu and had exactly 122, luckely, it unlocked itself on newgame+ with the number 15 in the painless dental cabinet
"Plasmids Are The Paint" Considering it's literally next to a tonic, i hardly believe i missed it.
I totally believe you though. Bioshock is super weird with its Difficulty Achievements that run counter to any other games that have similar Achievements. There a bunch of edits I need to do on this guide that takes the Difficulty swapping into account. Thanks for mentioning this as it further clarifies... that you basically just have to swap the Difficulty right at the very end of the game haha. It is good to know.
Little weird addition here, I started the game on Normal without reading the part about starting on survivor and I Never changed the difficulty. As a shot in the dark I changed it just before the last Adam Harvesting on Fontaine just to see, and it gave me the Survivor difficulty achievement. Not sure why but I'll take it! Saves me a play through
