All that after the reference pose apply for location the sandals just snapped the wrong, I had those on the main model for the entire time and Edit Mode do show how it should
Does that happen in blender, or after you're all done and the model is in gmod?
If its in blender, it could be all sorts of misalignments or even a bad rigging job.
When it happens ingame though, it could... still be a bad rigging job. But if the sandals are a bodygroup for example, you could have accidentally exported the sandals and the body itself with two different armatures. As in, export one, edit armature, export the other.
So i'd first try checking if the rigging on the feet and the sandals somewhat match up, and then export both the player and the other body parts you may have in one go. To prevent accidental mismatches of the armature
So basically all I did is port of Mamizou Futatsuiwa from Touhou (same one already on Gmod but doing it from scratch since I'm modder there too) and her sandals doesn't wanna line up with the feet which is rotated -12
But they might be separated because of the triscount. Unless its all bodygroups.

When i decompile the gmod playermodel, there are too many SMD files, from there are 48 parts of the SMD files, TsukinoBody.smd, TsukinoBackPack.smd, TsukinoCloak.smd, TsukinoEyeSphereNormal.smd, Etc. The monhun_tsukino_pm_physics.smd is for the base physics, How can all these SMD files be merged?
It's always been a separate addon, which is linked at the very top under requirements
You'd need to do a few debugging steps (undoing poses here and there until you find the cause) and then you can attempt to fix it.
But honestly, just redoing the pose from scratch with haggets tool will be quicker.
