A few things before we start...
I'm not a prolific guide writer. I'm not a particularly professional guide writer, so apologies in advance if you find this kinda messy or bothersome.
I decided to write this because I couldn't find an Achievement Guide for this game already. It isn't intended to cover anything besides Achievements, and even then I'm probably not going to go into vast detail.

I DID, on the other hand, find a guide for almost everything else in this game that helped me VASTLY in getting through it.
I would strongly recommend checking out THIS GUIDE[bkbrent.com] by BK Brent... as long as he keeps paying the server costs and keeping it online, at least.
While it seems mostly dedicated to advice on how to be the best possible player of what is, sadly, a mostly dead game... he does have pages for completing all the tricky quests and full details on all the items in the game and where to get them.
Expect me to reference it quite often... though I'm trying to avoid just repeating things he mentions in his own guide since he actually mentioned somewhere in the depths of it that he hates people taking his hard work... so... yeah...

That done with... there are three things worth keeping in mind while going for that sweet 100% achievement completion:

  1. This game is grindy. REALLY grindy. Many of the achievements are easy enough, but a small few of them are downright horrifyingly grindy. Classic Japanese game.
  2. 100% Achievements does not mean 100% Game Completion. This isn't one of those games that tags achievements onto basically everything you could possibly do in the game. Some things are needed, some things are optional (mostly related to titles), and some are completely unnecessary. A lot of this relates to the fact that this is a DLC-included full port of the game, and the achievements only relate to pre-DLC content.
  3. Nothing is missable. All Side-Episodes remain accessible until you access them. They won't go away from advancing the storyline, and no part of the game world is rendered inaccessible from progressing the game. SO don't ever feel bad about just playing this game in a relaxed fashion, or even completing the main questline blind first. Plus if you really want to relive the experience, New Game Plus is unlocked at the end... but there is really no need to bother with it unless you want to play through the main plot again...

I'll try my best not to spoil the plot for anyone who cares (I don't, usually)... but I expect someone might find something spoilerish in here NOT hidden behind a spoiler bar if they try... like the icon for the final achievement (it is in the last capitalised achievement section, if you care)... so be prepared for mild spoilerage potential.

Incidentally, I also admit that I cheat my ar$e off in games quite often.
You might disapprove of this sort of thing, but I game for enjoyment, and if something is getting boring and repetitive, or if I'm simply feeling more creative than the game allows, I take liberties. Game limitations should only exist to enhance enjoyment of a game, and if they're not doing that, I will break what I can. For instance, the fact that I use the Cheat Engine Speedhack function to speed up enhancing large numbers of weapons means I got the 100 hour playtime title despite only having played it for 89 real-time hours up to this point.
Still, I understand that a lot of players prefer to do things "legit", so I'll give advice for getting these achievements legitimately. And to be fair, I do most things legitimately... but yeah... I ain't gonna leave this game running overnight for a week just to get the 10-hour titles.

Anyway... on to the Achievements...
MAIN STORY ACHIEVEMENTS
All these achievements will be gained while playing through the main storyline. They're fairly self-explanatory and, assuming you intend to complete the game, you'll get them automatically.

The Invader
Cleared "The Invader."
The Impatient Skies
Cleared "The Impatient Skies."
Cold Trap
Cleared "Cold Trap."
Disastrous Moonlight

If you forget what you were supposed to do next at any point in the plot, keep an eye on the lower right corner panel in the pause menu for a rough idea... or go to all the accessible maps looking for the Yellow Exclamation Mark that indicates main plot progression.

Another thing that helps is to talk to NPCs in the Dicey Café. They usually drop fairly heavy hints about where you need to look next.

A few hints for certain episodes that gave me some trouble:
  • Episode 7 : You have to touch Ash Roller to progress, not just follow him. He goes around in circles.
  • Episode 8 : You're supposed to break the crystal of the colour corresponding to the colour of the legion whose emblem is shown... but yeah, you can just brute force it by breaking all of them and fighting off all the enemies IF you are quick / strong enough.
  • Episode 10 : The correct portal to progress the plot is the one at the end of the winding canyon just slightly southwest of the middle of the map, at ground level.
  • Episode 11 : That circle on the platform by the rock pond thing. You have to use a Sit Emote on it. I couldn't figure this one out without a guide. If you're using a Microsoft gamepad like I am, that would involve holding down the BACK button for a few moments then scrolling through to the emotes with RB or LB, then down to the Sit ones.
  • Episode 15 : You have to break those smoke-pots. There are a few on deck and most of them are inside the ship. The path through the ship is fairly linear, but you do have to jump over at least one barricade that looks impassable, and have to go up and down through holes in the floor / ceiling. Also note that the secret room at the end is only accessible if you're hovering or otherwise NOT on the ground, though you don't need it to progress the main plot.
  • Episode 17 : The final duel between Kirito and Silver Crow happens AFTER the end-credits, and unlocks New Game Plus. You can just keep playing the game without doing the NG+ thing though.
And for situations where you still aren't sure what to do, you could always check BK Brent's Story Mode Guide[bkbrent.com]...
SIDE-EPISODE / CHARACTER ACHIEVEMENTS
Most of these are advanced by following those little Green Exclamation Marks you can see about the place sometimes. They're basically side-quests, though some are just dialogue, and others that involve actual combat usually warn the player beforehand.

Most of these Side-Episodes require you be playing as a certain character when talking to the quest-giver... but they'll let you know quite plainly who they want to speak to, so it isn't exactly secret.
Most of them also unlock through Main Quest progression and some are only available in post-game (i.e. after the credits).

NOTE: Not all side-quests are required for achievements. You should probably be doing all the ones you can as some of them unlock characters (that can contribute towards titles), but many of them don't contribute to achievements directly.

Non-Mandatory Character Unlocks:
Argo, Sakuya and Alicia Rue are unlocked via side-episodes in late game, found in the Dicey Café. I believe anyone can speak to them. They have no associated achievements though.
Lux can be unlocked via side-episode in post game by speaking to her as Leafa, Lisbeth or Silica in town, but has no associated achievement.
Sachi and Heathcliff were originally DLC and are automatically unlocked from the start in the Steam version of the game.
Yuna was originally DLC and is unlocked via a side-episode in the Dicey Café available from the beginning of the game.
Persona Vabel was originally DLC and unlocks automatically in post-game after the credits.
Eugeo & Alice were originally DLC and unlocked via side-episode quests in post-game.
Oberon was originally DLC, was and is always a complete bastard, and is also unlocked in a side-episode quest in post-game... THEN has to be bought from the BP shop for 1BP if you really want him (I'd advise against it, unless you just want to steal his sword).

The conversations at the fountain unlocked by getting the Golden Handshake affinity marker against a character probably don't contribute to achievements... I don't think they do anyway... BUT they DO contribute to unlocking a lot of the Original Sword Skills for specific characters in the arena... so I'd advise doing them if you want your characters to be stronger, and for the Chain-Fighter achievement later. They're just not an achievement priority generally.

Memories of Pixies
Cleared every "Navigation" episode.
You get this one by following the Pixie side-episode chain to its end. The one that gives little pixie-stone accessories that SAO characters can use. Play as Kirito and look for SAO characters hanging around town or the Dicey Café wanting to speak to him.
Note that the side-episodes unlock with Main Story progression AND having completed previous side-episodes in the chain and you won't be able to complete the episode line and get the achievement until post-game.

Seeking Seals
Cleared every "Mystification" episode.
You get this one by following the Seal / Wing side-episode chain to its end. The one that gives little wing armaments that AW characters can use. Play as Silver Crow and look for AW characters hanging around town or the Dicey Café wanting to speak to him.
Note that the side-episodes unlock with Main Story progression AND having completed previous side-episodes in the chain and you won't be able to complete the episode line and get the achievement until post-game.

Seven Kings of Pure Color
Unlocked all seven Kings.
This is only an option after completing the "Mystification" episodes in post-game and getting the Seeking Seals achievement. After beating the Seven Kings at the end of the seal / wing side-episode chain, you should be able to purchase the dummy accounts for the Seven Kings at the BP shop.
The BP shop, if you're somehow unaware, is down the side-street to the right of the save point in town and to the left of the Dicey Café entrance + quest board.
If you don't have enough BP to buy all the Seven Kings, you CAN trade in monster materials... but I'd be wary of doing that as you'll need them for weapon upgrades and some of the Extra Quests.

Note: You do not need to pay for the other characters at the BP shop. They're entirely optional. ONLY the Seven Kings are achievement-mandatory.

Rainbow Songstress
Unlocked Seven.
In post-game, Seven should be visible hanging out near the fountain in town. I believe she needs Kirito to speak to her and unlock Seven (and Sumeragi). Simple enough.

Accelerated Leap
Unlocked Nitride Unica.
In post-game, Black Lotus will be hanging out inside the Dicey Café just next to the door when you enter. Speak to her as Silver Crow to get Nitride Unica. Easy.

You Said It, Miss Strea!
Started episode 5 of "Say it, Miss Strea!.
In the Dicey Café, in the corner furthest from the entrance, there is a really retro-looking television set that should be marked as "Gallery". You can click on that to view CG unlocked through side episodes AND a small selection of videos titled "Say It, Miss Strea!", in which Strea looks drop-dead gorgeous in glasses and a suit (unfortunately she can't ever wear this outfit in-game).
The 5th episode is the only one you need to watch (technically just start watching) for the achievement, and unlocks in post-game... so yeah, this is another easy post-game cheevo.



QUEST ACHIEVEMENTS (Part 1)
Just the basics here...
You might have noticed the quest board outside the Dicey Café entrance in town.
This is your source for all the Extra Quests and Challenge Quests in the game.
Besides the main story quests and the side episodes, there are two other types of quest in the game: Sudden Quests, which are activated and completed in-situ on the game maps, and Friend Quests which are functionally similar to the side-episode questlines and unlocked sequentially by speaking to specific characters under specific conditions.

So... the achievements...

First Challenge
Cleared a Challenge Quest.
Challenge Quests are just miscellaneous things to do while you're doing other things.
You can take up to 5 of them at a time from the quest board at a time. More of them unlock as you progress through the main storyline.
There are three types:
  1. Kill X number of Y monsters. X is usually 30, though it can be less for rarer monster types. Pressing the Y button while highlighting the quest will show you what the monster looks like. The quests typically unlock on the quest board shortly after the monster first becomes available, so if you're doing them all as they unlock, check the areas the main quest has just unlocked to find the monster type in question. Note that monsters you need to kill for a Challenge Quest will have a tiny yellow triangle over them on-screen, which can be useful for identifying them.
    If all else fails, check BK Brent's Mob Monster List[bkbrent.com] to find out where the damned things are hiding.
  2. Collect X number of Y monster drop items. X is usually 5, though sometimes 3 for rarer drops. This can be an absolute pain to figure out, but if you just kill every monster you come across you'll probably get at least one of each item eventually, and then the item description will tell you what they drop from so you can go farm more. I'd strongly advise checking BK Brent's Material List[bkbrent.com].
  3. Go to point X on the map. You get a very rough explanation of where you need to go, and then you go there, find a shiny spot and examine it to fulfil the quest requirements. Apart from the first two, the descriptions of the places you're supposed to go to are absurdly vague to the point of near uselessness. Again, BK Brent has done all the hard work so you don't have to[bkbrent.com].
For the record, you only need to do one Challenge Quest for this achievement... BUT there are titles linked to doing vast numbers of different ones... so you might as well get used to doing them.


Let's Go Boss Hunting!
Cleared an Extra Quest.
Extra Quests are basically just fighting and killing a boss that you've fought before. There aren't any available at the beginning of the game. You should get your first one unlocked after completing the first Friend Quest for Amelia in Welgunde as part of the main story.
The rest will unlock sequentially as you progress the main story, side-episode questlines and Friend Quests... though some of the later ones are unlocked by completing other Extra Quests first.
You only need to complete one for this particular achievement, but there are other achievements pertaining to Extra Quests and there are also titles for doing a certain number of different ones (also a title for soloing them).


Surprise!
Cleared a Sudden Quest.
The game should give you a tutorial on these things.
If you go to certain areas on one of the open maps (or in Otherworld maps), a Sudden Quest will start, requiring you to do something specific within a rather strict time-limit and a rather strict area (a red ring will mark the boundary of the quest on the map). If you leave the quest area, the quest will cancel... and no, you can't just repeat it. It might respawn later. If you're anything like me, you'll end up flying through a few of these while zooming around the map and will only briefly see SUDDEN QUEST and QUEST CANCELLED or whatever flash up on the screen while zooming about... mostly because they're usually invisible until you stumble across them... yeah.

BUT WORRY NOT... There is a really easy way to find these things: Certain SAO characters (e.g. Kirito, Philia, Rain) have a Support Magic skill called Searcher. There is also an equivalent Support Incarnate skill held by a few AW characters (e.g. Scarlet Rain) called Extend Vision that does exactly the same thing. These skills, when cast, cause two changes to the area map: They make treasure chests visible in the form of little yellow chest icons... AND they make Sudden Quests visible, in the form of yellow rings of various shades and sizes on the map.
Basically... fly into the yellow ring at the right altitude and the Sudden Quest will start. Judging the right altitude can be tricky, and the map will show you Sudden Quest rings at altitudes you might not have unlocked yet ( beyond the flight limit / sky barrier on any of the maps, or underground in Flosshilde)... but roughly speaking the colour intensity of the circle indicates how close you are to the right altitude. Also you can judge where some of them are if they're on floating islands in the sky as the circle will almost exactly contain the island.
Specific rings in specific areas always yield the same type of Sudden Quest, though monster types may vary.

QUEST ACHIEVEMENTS (Part 2)
... ( Sudden Quests continued. )

Anyways... the Sudden Quests themselves come in a few types:
  1. Kill all the targets. A fixed number of monsters of a certain type will appear as larger red marks on the map and you have to kill them all before the time runs out. Can be trickier than it looks if your damage output isn't high. The monster type isn't always the same for the same area, and they're not always monsters endemic to that map either.
  2. Kill as many targets as you can within the time limit. Similar to the first kind, except monsters of a certain type respawn infinitely until the time limit and you have to kill as many as you can. It flashes up on the screen when you get 20, 40, 60, 80 and 100 kills respectively. Like the other monster killing quest, the ones for this type are not always the same type every time you get the quest, and nor are they necessarily endemic to that map. This type of Sudden Quest is an absolute godsend if the monster type matches a Challenge Quest you have active, or if you're trying to get the title for 300 of a monster type... OR if you want Rare Drops from the monster type in question. It is best to try and memorise where these Sudden Quest types spawn and farm them for all they're worth.
  3. Collect Lost Souls. i.e. Fly through 5 glowing points in the area. Basically like a flying race test, though most of them are really easy in practice. You can see them marked with yellow spots on the map, AND they're marked with triangles on the screen that you can see through obstacles, so there is no real excuse for not being able to find them. The biggest threat to these Sudden Quests isn't the time limit, but accidentally flying out of the quest area. Note also that many of these will spawn additional monsters in the area that might not normally be commonly found there, so if you've got enough time, you might want to kill a few along the way (helps towards 300-kill monster titles and certain Challenge Quests).
  4. Get a certain Combo Count. A bunch of monsters will spawn of a single type, but they'll be invincible and exist just for you to beat on repeatedly. Wail on them with your best skill connect combos to up your combo count... OR just spam Wind Cutter (tier 1 wind element spell) for easy victory.
  5. Protect the NPC. Oh joy. These ones. An NPC appears as a white mark on the map, and gets mobbed by enemies of a certain type, as well as other random monsters in the vicinity, and you have to protect them until the time runs out. This is mostly luck-based. If the monsters that spawn are those relatively passive jellyfish-type monsters that float about and don't do much, this is a cake-walk. If, on the other hand, the monsters that spawn are harpies that delight in ranged attacks... you're gonna have a bad time. My advice: stay near the NPC, draw aggro to yourself if you can, and attack anything that comes near the NPC. Not sure if you can heal them or not, but your own AI team-mates never do... so... yeah.
  6. Beat the Boss. All the native monsters in the area despawn and a Boss monster spawns nearby (not immediately on top of you, thankfully). You gotta kill it within the time limit. This type of Sudden Quest doesn't become available until quite late in the game, storywise. Or rather they're mostly in areas not immediately accessible from the beginning... because if you're unprepared, this sort of thing can wreck you. A strange side-effect of this sort of Sudden Quest though is that if you manage to kill the native monsters in the area before triggering the quest, they'll all respawn as new after the quest if you complete, fail or cancel it.
Enough about that.


A Friend in Need
Cleared a Friend Quest.
You should get this achievement just by progressing the main story.
All Friend Quests form part of 4 questlines:
Amelia's Quests (Cait Sith girl with pigtails that wants you to find treasure for her),
Hexe's Quests (lady in the arena who wants you to decontaminate cubes by doing Sudden Quests in the different map areas),
Santa Maria Quests (Santa is that bronzey goldy looking dude with horns you see about the place in map areas, while Maria is the black-dressed skinny lady with a ponytail who can be found in town or the Dicey Café, and their questline starts in the late game and revolves around unlocking the Otherworld Portals).
Kumahachi's Quests (A burly guy who shows up in late game down the same alley as the BP Shop and requests you go fetch things for him from the various maps.)

Only Hexe's quests pertain to an Achievement of their own (which I'll get to in a moment), but you will probably want to do the Santa Maria ones at the very least as you can't complete the Hexe quests without doing so... plus the others for Title reasons... and also the once-per-playthrough Crest of Yggdrasil accessory you get from completing the Santa Maria questline.
And yes, for all these quests, you can follow BK Brent's Friend Quest guide[bkbrent.com]. You'll probably need it to figure out where the various monsters are...


The Mediator
Completed all of Hexe's requests.
You get this achievement for completing the Hexe Friend Quest chain.
This mostly involves doing a lot of Sudden Quests, so reference what I said earlier about those.
After first talking to Hexe and being shown the contaminated cubes floating around in the arena (accessible from town), she'll tell you to go do Sudden Quests in each area.
SO you go to the map areas, activate Searcher / Extend Vision, and do 15 Sudden Quests (you can do more now if you want and it will count for later). After 15, Hexe will send you a message in town asking you to come talk to her, and then give you a quest to beat a boss in that map area. So you go and beat up the boss (finding it can be a pain, but there are other guides for that, including BK Brent's one referenced earlier), then report back to Hexe... THEN you go and do another 15 Sudden Quests in that area (to bring the total up to 30), and Hexe will give you another message in town to go to her for a reward.
The 5th cube in the Arena pertains to Otherworld maps. i.e. the Accel World maps you unlock via the Santa Maria questline (go look for that bronze dude Santa near the teleporter in Woglinde near end-game to get started). All of the Otherworld maps share a single cube, so you can get your Sudden Quests across any of them and they all count... BUT be warned that they're generally tougher than the ones in any other map area. The boss quest for the Otherworld maps is located in the post-apocalyptic city stage you access from Woglinde, and will be right in front of you when you enter.
Once you've got the 30 Sudden Quests and the boss beaten AND the reward for each of the 5 cubes, Hexe will give you one last task: A boss fight... against Melancholia... one of the toughest mofos in the game (and a palette-swap of a certain other boss). Once you've beaten that guy though, speak to Hexe again and you'll get this achievement.


Bounty Hunter
Cleared 50 quests.
As the description indicates, clear any 50 quests (Challenge, Extra, Sudden, Friend) to get this achievement. I don't think they have to be unique quests, but it is probably more interesting to do different quests than just to spam the same thing 50 times.

QUEST ACHIEVEMENTS (Part 3)

Black Blades, Silver Wings
Cleared an Extra Quest with only Kirito and Silver Crow.
The achievement description explains what you have to do... but in case it isn't blatantly obvious...
Change your party to just Kirito and Silver Crow, nobody in the 3rd slot, select any Extra Quest (including ones you've already completed, and yes, it can be the easiest boss in the game again), go into the teleporter and beat the boss.
Achievement Get.
It is that easy.


Berserk Healer, World's End
Cleared an Extra Quest with only Asuna and Black Lotus.
As with the previous achievement, it is fairly obvious what you have to do from the description.
Change your party to Asuna and Black Lotus with nobody in the 3rd slot, pick any Extra Quest (completed or otherwise), step into the teleporter and beat the boss.
Easy.


The Elements
Cleared an Extra Quest with only Sky Raker, Ardor Maiden, and Aqua Current.
The third character-limited Extra Quest achievement. This one requires your party to consist of Sky Raker, Ardor Maiden and Aqua Current, obviously. And due to when you unlock these characters, you might have to wait a bit longer for this achievement. Still, not a difficult one. I'd personally recommend playing Ardor Maiden since I far prefer ranged combat... but play whichever you're most comfortable with. Just load them all into the party, pick an Extra Quest (again, any will do), teleport, kill boss. You know the drill.

WEAPON / BLACKSMITHING ACHIEVEMENTS
This is where the grind really starts, ladies, gentlemen and otherwise identifying...

Use the Forge!
Employed a blacksmith's services.
The easy achievement. Also the mandatory Star Wars reference for no particular reason.
Go to the Blacksmith in town (to the left of the shops, and to the right of the path leading to the arena), then get Liz (or Rain, once she is unlocked) to Enhance, Synthesize or Transform a weapon.
I'd recommend just Enhancing the physical aspect of one of the basic weapons you start with for this achievement since it only takes a bit of Copper that you can get from Kobalds in Woglinde or buy at the Materials shop. Of course this is just the beginning...
You'll want to hold on to at least one of those starting weapons. Trust me on that.


Ultimate Masterpiece!
Enhanced one weapon to +10.
To get this achievement, you need to Enhance any weapon (via physical or elemental upgrades) from 0 to +10.
And to do THAT you'll need to upgrade the Blacksmith Level to 10 by smithing lots of things. If you think that is grindy, we've not even begun to grind yet. At least you don't have to upgrade both Liz and Rain since they both share the same Blacksmith Level.
Anyway, you can't Enhance a weapon to a higher level than your Blacksmith Level as a general rule... so upgrading the Blacksmith and Enhancing a weapon go hand in hand in that respect. I'm not entirely sure whether the Blacksmith gets more experience for upgrading higher-Rank weapons or not... but I got my Blacksmith to level 10 mostly by upgrading every single trash weapon drop I got as far as I could with the materials available. That meant an awful lot of Copper spent upgrading trashy Rank 1 weapons up a few numbers.
Once you're done upgrading trash weapons and have a nice, shiny Level 10 Blacksmith... that enables you (with the right materials) to upgrade a weapon to +10.
Congratulations. You're still not even 10% of the way towards the fully upgraded weapon.


Max Enhance
Raised a weapon from Rank 1 to Rank 10.
This is one of the rarest achievements in the game for good reason: GRIND. Material Grind, primarily.
Once you've got the previous achievement for getting a +10 weapon, you will need another copy of that weapon to Transform it... which is to say raise its rank. The process is simple: Get a +10 weapon, get another copy of the weapon (any upgrade level or rank of that weapon will do, but it has to be the same weapon, so I'd recommend a completely non-upgraded one). Select the weapon you want to upgrade THEN the weapon you want to use as fuel (the second weapon will be destroyed in the process), and the first weapon will gain a rank and lose its enhancement.
e.g. Divination +10 / Rank 1 becomes Divination / Rank 2.

To get this achievement... you need to start with a Rank 1 weapon, Enhance it to +10, Transform it to Rank 2, Enhance it to +10, Transform it to Rank 3, Enhance it to +10... etc... all the way to Rank 10.
Obviously this takes a lot of materials / monster parts of ever-increasing rarity all the way up to rare end-game boss drops towards the end. So expect to have to murder a lot of monsters, including bosses, with drop rate items (like the Lapis Ring) equipped. I'd also recommend a high combo count and natural drop-boost passive skills where possible (e.g. play as Philia).

I would STRONGLY recommend getting an Elemental version of one of the Rank 1 weapon types you can buy in the shop... like the Two-Handed Sword or Target Lance. That means not using the weapons you actually start with, but getting one from a treasure chest or monster drop in Woglinde.
The reasons for this are twofold: Elemental weapons can have either the physical aspect or the elemental aspect Enhanced, so you have twice the upgrade options if you don't have the right materials (whereas you'd only have one upgrade path if you went with a non-elemental Divination), and as long as you can buy additional copies of the weapon from the shop, Transforming it to raise the rank is easier.

Anyway... the achievement will pop when that weapon finally gets Transformed to Rank 10.
You don't need to enhance it any further for the purposes of this achievement... BUT if you actually intend to use the weapon (you probably shouldn't, since weapons with a higher starting rank have naturally stronger power / element / skill boosts), you can continue to Enhance + Transform it up to the in-game maximum of Rank 12 / +10.
And yes, the achievement name is a misnomer now. Rank 10 used to be the cap before patches and DLC in the playstation version raised the cap to Rank 12... which we get all-included in the Steam version.


Weapon Collector
Obtained 100 different types of equipment.
This one should be easy enough that you don't have to specifically aim for it... assuming you're aiming for Title completion and kill loads of monsters (with all the usual Rare Drop boost bonuses possible).
I should probably mention here though that killing bosses in Extra Quests has a considerably lower Rare Drop loot rate than killing them anywhere else... so if you're thinking of just spamming Extra Quests, I'd only suggest it if you're after a very specific weapon and not just trying to get rare weapon drops in general.
Play as Philia a lot, wear a Lapis Ring, use Searcher, open treasure chests, do monster-spam Sudden Quests.. and you should get this achievement without even realising it.

Note that Accel World equipment counts towards this achievement too.

Also... for no particular reason... Sinon Best Girl Achievement Icon BANZAI! She looks best with her glasses on though.
EXPERIENCE GRIND ACHIEVEMENTS
Here comes the fun part... not.

Skill Expert
Raised one SAO character's skill level to 500
Each skill in the game has a maximum level of 1000. Thank the gods of grind you don't have to go that far. You do have to go to 500 though, which is numerically half way and, usage-wise, less than that.
To raise a skill level, just use it. Skills gain experience based on how many times they trigger on a target. Attack skills need to hit an enemy to gain experience. Support skills need to trigger on an ally to gain experience (which means that you'll get 3 times as much experience if it is something that buffs the entire party).
Debuff skills are probably the best idea here, since Debuffs in this game have absolutely ungodly range and will debuff every single enemy within line of sight in all directions... and you get experience for every single one. Stand around somewhere flat like the Flosshilde underground with Kirito or Philia and spam Weaken, and the skill will gain absurd amounts of experience relatively quickly... Just keep an eye on your MP though.

Also note: Items that boost Experience gain ALSO boost skill experience. That means it is worth having the Crest of Yggdrasil equipped before you start skill-spamming for this achievement.


Master Technician
Raised one AW character's skill level to 500
You could be forgiven for thinking this the same thing as the SAO character skills.
But there is one rather notable difference here: Accel World characters are PvP 1-on-1 focused. AW characters don't get huge map-spanning Debuff magic, nor even party-wide buff skills. Their buffs are limited to themselves. This seriously limits how many targets they can hit with a skill simultaneously, and thus the experience gain.
Your best bet for this achievement is probably to go as Scarlet Rain, Sky Raker, Ardor Maiden or Nitride Unica and spam big a big AoE attack as much as you can on dense groups of enemies. Sudden Quests that spawn tons of enemies within a time limit can help with this.
Alternatively... just play as the same AW character for most of the game and bias one AoE move above all others, and you should get this eventually.

Again, Crest of Yggdrasil is your friend here. Exp boosts help raise skills too. Don't forget.


Mid-Level
Raised one character's level to 500.
Highest Level
Raised one character's level to 1000.
I'd just like to stop here to recount a little anecdote...
When I completed the game, I noticed the last few characters I unlocked popped in at about level 600. The implication being that level 600 is about what you should be at for end-game.
This surprised me, because my average character level at this point... having spent BP on evening out the lowest-levelled characters, was around 200... and my absolute highest-level character (Sinon, obviously) was around 450.
No wonder the end-game had been so difficult for me. Oh well.

Anyway... get that Crest of Yggdrasil (or, failing that, a Glow Amulet from those Sky Prowlers, wasn't it?) and grind grind grind. Kill everything then kill it again, and again, and again. Chances are you'll eventually get these if you play one character all the damned time at the expense of everyone else... especially if you're trying to grind out titles.
ALTERNATIVELY, if you've got a ton of spare BP or spare monster drops you don't explicitly need for weapon upgrades... just binge BP to raise a character's level up at the BP shop. Watch your Accel World friends blanche in horror as you use up their precious burst points...
But expect this to take a long time either way.

Also... another misnomer there on the Highest Level achievement, since again patches to the original game brought the character level cap up to 1200... not 1000... and there are titles for each non-DLC character reaching level 1000 (and a few for numbers of characters brought to level 1200)... though I'd only regard collecting those as an absolute last resort.


TITLE ACHIEVEMENTS (Part 1)
The One of Many Names
Received 50 titles.
Hundred Holder
Acquired 100 total titles.
... Yeah. See, these achievements are deceptive... because earning a Title in this game generally requires doing things which, in any other game, would warrant achievements in and of themselves.
So getting the Hundred Holder achievement is, in and of itself, equivalent to getting 100 achievements. That is why it is the second rarest achievement in the game (behind the standard 100% Completion achievement for getting all the others).

Before you start recoiling in horror... keep in mind that there are over 150 titles to get, and you only need to get about 2/3rds of them. Any 2/3rds. So you can pick and choose what you want and what you don't.
Keep in mind that even though it doesn't tell you what the actual Titles are before you get them, you can look up the rough requirements for achieving the Titles in the Title section of the pause menu at any time... so you know what you have to do for most of them in advance.

I guess I had some grandiose notions about expanding this section into a super exhaustive Title Guide, but I'm not sure I can be bothered to go that far any more given I'm all but done with this game. I might make a separate title guide at some point, but I also might not...
STILL... I'll do my best to at least cover the best ones to procure...
  • The first few Titles correspond to time spent playing the game. Overall (1 to 3), and then in particular map areas (4 to 7). Now, you might get some of those just playing the game normally given how much grinding you will have to do for weapons, skills and levels...
    The total playtime titles run when the game is paused, or in town... so not a problem there.
    I believe the exploration titles for the different maps require being unpaused though, so if you plan on leaving the game running overnight, I'd recommend finding a safe place to idle.
    Later on there is also a Title (73) for 10 hours spent flying. I wouldn't even bother unless you can rubber-band your controller to fly in circles without bumping into anything.
  • The next bunch of Titles relate to quests. You've got one for soloing 10 Extra Quests (8), which isn't too bad if you just do the weakest ones over and over again with an End-Game character. Most of them (9 to 18) involve doing a certain number of distinct quests of various types, so not just repeating the same one over and over again. These should be gained easily enough through the course of the game.
  • Another bunch of Titles (19 to 27) correspond to Multiplayer. I skipped these entirely. If you can find anyone to play with, these might be another easy win for you.
  • The level 1000 Titles (28 to 65) . Fun times. There is one per character for all the non-DLC characters. i.e. Don't bother getting Sachi or Persona Vabel to level 1000, as you won't get anything for it. I'd consider these titles a bit of a last resort... BUT you will get at least one for the character you got to level 1000 with for the achievement (probably Kirito). Your choice. It is a lot of titles to completely ignore if you choose not to.
  • Misc Combat / Skill Titles. These vary massively. Some are easy, some are hellishly difficult. I'll try to break them down for you. Nested List Time!:
    1. Accel Move Attacks (66) : Use an AW character, press the jump + attack button to close with the enemy, and make your first attack on them hit. You should get this automatically if you play AW characters a lot.
    2. Skill Connects (67) : Use a combat skill immediately after another combat skill to get a skill connect. You get a damage bonus and can chain skills together. Should get this automatically since skill-connects are practically mandatory for high damage against bosses.
    3. Switch Attacks (68) : It says "Switches", but it means Switch Attacks, not just pressing the shoulder buttons. Press the strong attack and dodge buttons together when adjacent to an enemy with a full switch bar (pink, top-right corner of the hud) to do a Switch Attack and get some bonus damage. Not too tough to do a bunch of times.
    4. Damage in a single attack (69 to 71) : Should get these by the end of the game automatically. If not, then you should get them while trying for the Chain-Fighter achievement. Or just use Sinon's sniper rifle OSS (Best Girl).
    5. Die a lot (72) : ... Yeah. Another automatic one unless you're godly at the game. Always remember to bring a healer with you, and Yggdrasil items.
    6. High Jumps (74) : Skipping 73 since that is the 10 hours flying one... This is another Accel World character Title for doing the high jump move (hold down the jump button until the character flashes red, then release). You'll probably get this automatically if you play AW characters a lot.
    7. Combos (75 to 77) : The first two are pretty easy. The third one... spam Wind Cutter. Really. The Wind Cutter spell is easy mode for combo counts. Any Sylph should have it.
    8. 30 Consecutive Just Guards (78) : This is what separates the casuals from the veterans. Dark Souls players should know how this goes. Anyone can block attacks (run + dash buttons together, ironically), but if you do it JUST before a physical attack hits, you'll get a yellowy green flash instead of a dull bluish one. You need to do 30 of these in a row... without being hit nor doing a regular guard inbetween. And yes, I got this one out of sheer stubborn determination.
    9. Attack Skills (79) : Automatic. Seriously. Unless you don't use attack skills, you'll get this without trying.
    10. Counter Attack Skills (80) : Not as tricky as 78, but not exactly clear what it means early game. Some weapons / characters get attack skills whose icon looks like an arrow curving around to the right (e.g. Specula Cross for Dual Blades, Vengeance of the Lotus for Asuna). You need to use these in the same scenarios as you would be blocking for title 78. Just as a physical attack is about to hit, use the skill to deflect and counterattack. Note you can only use most of these (besides Chrome's one) for a Skill Connect if it actually triggers properly.
    11. Healing Skills (81) : Play Asuna, spam Healing on allies 100 times. It counts once for each ally, so 100 casts should be enough if everyone is within range.
    12. Assist Skills (82) : Use Support magic that targets the whole party (e.g. Quick, Sharpen). Spam it on the party 100 times. Title get.
    13. Items (85) : Probably another automatic one, especially if playing AW characters. But still, use a lot of consumable items. Doesn't matter which.
    14. Destructible Objects (86) : Break barrels, break boxes, break random stuff in Otherworld stages. Keep vandalising the world like you know you are already and you'll get this.
    15. Starburst Stream, DBs version (89) : Use Kirito or Rain, spam Starburst Stream on monsters a lot. Black Lotus's version doesn't count.
      Okay... enough of the misc-combat section...
  • I honestly don't know what titles 83 and 84 are. Never figured it out.
  • Titles 87 and 88 are literally the easiest titles in the game. Just spam the Wave and Grovel emotes respectively anywhere until the titles proc.
  • Four titles (90 to 93) correspond to Otherworld maps unlocked via the Santa Maria Friend Quest chain. You have to visit each map 10 times in total. I believe the times the story takes you there beforehand count towards the total, thankfully. Not too difficult, though kinda repetitive.
  • Visit all the dungeons in the game for title 94. You won't be able to get it until post-game, but you don't have to complete the dungeon. Just step in, step out.

TITLE ACHIEVEMENTS (Part 2)
... (continued from where I left off)
  • Blacksmithing Titles (95 to 98). You should get these in the course of getting the smithing achievements. The only one not covered at all by that is the Synthesizing one, but you'll probably be doing that just to strengthen your characters during the game anyway.
  • Cash titles (99 to 102). More automatic stuff, if you're grinding for other achievements. Money gain shouldn't be an issue, and spending the money should come naturally for items and weapon transformations for Yuld, and unlocking the seven kings / levelling up characters for BP.
  • There are two Titles for treasure chests. Regular ones (103) should be easy enough if you're using Searcher with Philia or Kirito a lot as I suggested before. The "Pure Color" chests might require a little more searching. There are a few around town, and the rest can be found in dungeons and do not respawn like regular chests. Still, if you're doing all the dungeons after getting all the coloured orbs, this should be easy enough.
  • The next 3 Titles 105 to 107) are for collecting quantities of equipment. I don't think it matters if they're duplicates in this case. If you're collecting loads of weapons and whatnot, it should be automatic.
  • Titles 108 to 118 are for getting Rank 10 versions of every SAO armament (including Shields and Wands). It can either be Rank 10 naturally or be raised to Rank 10 by smithing. Either works. Wands are the only ones you're not going to see much of dropping from Monsters, but I believe one of the later Friend Quests gives one as a reward, so that is that sorted.
  • Lots of Monster Titles. Title 120 is for 10000 monsters, Title 121 is for 300 "Rare Monsters" (not sure what does or doesn't constitute rarity, but it wasn't difficult to get). Titles 122 through to 145 are for killing 300 of each regular mob monster class in the game, with the exception of Mimics, for which you only need 100. Note that any monster with the same basic model will count towards the title (e.g. Orcs, Goblins, Orc Berserkers, etc all count towards the Orc title). The most annoying ones are probably Worms (those really long flying centipede things) and Mimics (fake chests with legs)... but you can grind both of those out outside and inside of that dungeon up near the tree in Woglinde.
  • I'd maybe go for Title 146 (get 1 character to level 1200) since you're going to have at least one at 1000 anyway, but I'd give the other two (10 and 30 characters respectively) a skip if you can. Incidentally, I believe this is where post-patch / DLC titles start...
  • The Infinite Transition Dungeon titles are from 149 to 155 and form the last titles of the game. 149 and 150 are for multiplayer, so I skipped those, but they involve things unlocked by reaching certain floors in the dungeon. 151 is for 20 Enigmas (given some of them involve standing still for 15 seconds, collecting a single Lost Soul which shows up on the map, or just breaking coloured objects, this should be easy enough). And finally, 152 to 155 are for reaching specific floors in the infinite transition dungeon. 10th floor you'll get automatically if you're unlocking all characters, but the others are... you guessed it... a grind.
Phew... finally.
Title blather over. Hopefully you'll be able to select 100 that suit your purposes... but expect a lot of grind regardless. Not everything can be spamming 30 emotes.
MISC / OTHER ACHIEVEMENTS
Righteo... time to wrap up the remaining achievements that don't really fit elsewhere.

Don't Spend It All in One Place
Used BP to buy something.
In other words, use the BP shop.
As I already explained earlier, the BP shop is to the right of the save / multiplayer portals and to the left of the front of the Dicey Café (facing it).
You can go there to use BP to power up AW character's fixed attack weapons, rank up their armaments, unlock certain dummy accounts (for the Seven Kings and a few other characters) once you've gained the right to do so... AND to level up characters. So yeah, you'll probably get this in the course of getting other achievements or titles.


Switch!
Performed a Switch.
It means a Switch Attack... not just Switching characters via the shoulder buttons.
This is the first achievement most people get, since it is covered in the tutorial... but as I mentioned in the Title section, just fill up the Switch gauge, hit an enemy with the Y+B attack (strong attack plus dash) and pop goes the achievement.


Get the General!
Defeated the aura-clad monster general.
I'm sure you've seen one of those monsters with a reddish black aura that buffs the other monsters around it. They're everywhere. Once you kill one of those, this achievement pops.
Easy as pie.


Chain-Fighter
Chained together 8 skills with Skill Connect.
Oh lawdie... this one.
Okay, so I did get this automatically late in the game... but you will need a few things in order to get this, most likely:
Firstly, you'll need to have unlocked almost all of a character's skills for a particular weapon. You'll need 8, as the description states. Having a character's Original Sword Skill unlocked helps too.
Secondly, you'll need all the attack skills to be ready... not cooling down.
Thirdly, you'll ideally need to work out a sequence to ensure you can fire off all the attack skills at enemies back to back in an uninterrupted chain without the enemies all dying off half-way, whiffing any attacks, nor being bashed out of it by all the aggro you're pulling. I would definitely recommend boosting your Cooldown and Stagger Resistance for this.

Personally, and completely to nobody's surprise, I used Sinon (Best Girl, as previously noted) for this. Her Bow skills are excellent for chaining since they're long-range and thus enemies have to move pretty damned far away to get out of range... plus her Hecate 2 Sniper Rifle OSS chained onto the end of a skill-connect combo can smash an entire health-bar off even end-game bosses with the right buffs and elemental advantage.
If you can get to that delicious 180% Skill Connect bonus, you've definitely got this achievement.


Otherworld Wanderer
Destroyed a solid Otherworld object.
Kinda funny how Kuroyukihime goes on about objects in Otherworld AW stages usually being indestructible due to being based on real world objects, and how Blue Knight's incarnate ability is so special because he can cut through them...
... and then you do the Santa Maria questline, get into the World's End stage and start smashing cars... and this achievement pops.
Yeah. This is like smashing barrels in Woglinde, except you're smashing trash lying about in the Otherworld stages. Far easier than the plot suggests... but you've got to get there first.


Millennium Conquerer
Obtained all achievements.
This one will proc when you get the last other achievement (most likely Hundred Holder).
CONGLATURATION!!! A WINNER IS YOU!. You've 100% Completed Accel World VS Sword Art Online!!


Closing Comments...
Okay... so I just spent hours writing this with RSI in my fingers because I thought this should have existed when I first started playing the game.

And even though this is a port of a console game, completely non-canon even to the separate SAO game-canon which it is derived from... and practically dead in most ways... I STILL hope this extremely late guide can be of some use to someone somewhere.


Turns out most of it was stating the obvious... or just plugging BK Brent's game guide[bkbrent.com].
SPEAKING OF WHICH... go and give BK Brent all of your money. It keeps his guide running and available to all other players of this game.
And perhaps give some of the other sections of his page a read too. The stuff about Super Characters was kinda interesting... (though that said, he loves Two-Handed Swords a bit much for me, as I'm very much a Bow-user).

Can't remember now, sorry. But it'll definitely be in the portal where you pick the team members. I distinctly recall there being the option to go out solo, so going out with 2 should also be fine, though I don't recall ever doing it.
