Players who use this playermodel are almost always server admins, by virtue of said playermodel being the most visually distinctive out of everything else in the base game. You'll see them do typical admin stuff like float ominously around the map while watching players like a hawk, hold a physgun 24/7, and noclip into view out of the blue every once in a while.
If they aren't admins, Combine Elites will be ordinary players who think the playermodel looks really cool and badass, and will thus be rarely seen without it. They tend to be mediocre at combat in PvP sandbox servers if I remember correctly, if not moderately decent.
======== Dr. Kleiner ========
The signature PM of newbies who just joined Gmod for the first time, as well as the much-maligned Mingebag stereotype. In multiplayer Sandbox servers, Kleiners that you may come across are very often younger players who, at best, will go around spawning balloons everywhere, attaching thrusters to everything, or just blowing off steam by spawning NPCs and then murdering them, etc. At worst, they will deep-throat the mics so everyone can hear their pre-pubescent voices in their full, ear-destroying glory, use cheap, one-hit-kill weapons such as the RPG and the crossbow to harass others and compensate for their lack of FPS skill, push people around with props, and downright slowing down the server to a snail's pace by flooding the map with props (usually gigantic ones) and NPCs for shits and giggles, among others.
People who use Dr. Kleiner with a dark blue labcoat are insignificant, little sh*ts.
