#1 - Install your mod

#2 - Go into your RTW directory (wherever you installed the game), and look for the mod folder (eg. for Europa Barbarorum it's the "EB" folder)

#3 - Keep that folder's name in mind

#4 - Go to your Steam Library, right click Rome: Total War, and click Properties

#5 - Click on "Set Launch Options"

#6 - Type this in there (replace *MOD FOLDER NAME* with the name of your mod's folder)
-mod:*MOD FOLDER NAME* -show_err

eg. for Europa Barbarorum it would look like
-mod:eb -show_err

#7 - it's all set.

well not all mods come with install instuctions

There's no folder called "Mods" like for most games, the root directory ("Rome Total War Gold" folder for you porbably, in steamapps > common) is where the mod's folder should be (if you've already installed it properly, and mods always come with installation instuctions)

Haven't found the mod folder would it work if I created the mod folder

Can you help with the total realism mod, i have downloaded latest version however not sure on the next step /:

Ah. Well here's a link to Total War Centre's Rome:TW hosted mod section (hosted mods are the big ones that deserve their own sub-forum) http://www.twcenter.net/forums/forumdisplay.php?1989-Total-War-Rome-II-Hosted-Modifications . Europa Barbarorum, Roma Surrectum, Rome Total Realism are the 3 most popular ones I know of.

New to the modding lark so if i can ask, where do i get the total realism mod i thinks its called and how do i install it ?

Maybe ask on their own forum. Probably have a better chance of finding someone that knows.
