1. An enemy you killed before reaching the checkpoint respawning when they're not supposed to, or forgetting to kill one after multiple resets.
2. A result of enemies being located in places you would never actually go in the course of a mission, thus resulting in an incomplete kill count. (For example, guards in the beginning of the first mission, which you'd have to backtrack extensively to kill.)
3. Or it could be a result of enemies being distracted or killed as a side effect of attacking other enemies, and the game changing the bonus or discounting it entirely. I definitely recall there were times when the bonus score indicators were potentially inaccurate, especially with a non-lethal runs and/or indirect kills.

I have a question about the screen before the score bonus screen where it shows each enemy with "nothing", "?", "!", or "x" individually. The symbols don't seem "add up" right. For example, if I kill every enemy on a level I would expect them to all have an "x" based on your description, but some (two or three) will have "nothing" or a "?" even though most enemies were "undetected" and "distracted" before I killed them. Either there should be many more "nothing" and "?" for all the enemies that were "undetected" and "distracted" or there should be none at all since they were all killed.
