Compile the Source model, in the same manner as if the model didn't have any physics mesh for it. It gets "converted" into a PHY file at the same time as the rest of the QC, DMX, SMD, et cetera files get compiled into MDL, VVD, and VTX files.

Make a mesh object (or several) where each batch of vertices is only attached to a single bone, preferably with a low-ish polycount, then export it as a DMX or SMD mesh.
Feel free to import a ragdoll physics mesh (for example from a de-compiled Source model) to see what sort of design you could try to make it look like for it to work right.

Then, if it's for a single-bone physics mesh, load that DMX or SMD in the QC file using $CollisionModel. If it's for a multi-bone ragdoll thing, use $CollisionJoints instead.

In the "Source Engine Exportables" properties panel thingie of the Blender Source Tools plug-in, select the collection, then in the "Group Properties" section, enable the "Suppress" tick-box to make all exportable objects of that collection export individually.

What is the problem, exactly? You've stated some information (though lacking the QC file's contents), but nothing suggesting what's wrong, nor even directly if anything is wrong.

It might help Artfunkel diagnose the problem if you'd provide a Blender session (and external flex controller definition file, if it's not embedded in the session) where the problem occurs.

What "doesn't work"? I never suggested anything that should fix it, I only asked you to import the exported files into a new Blender session to verify that everything is correct in the exported files, to which "[it] doesn't work" doesn't make sense as an answer/explanation.

Then the head doesn't have the same skeleton/armature as the body mesh file and/or the animation file.
Try importing the exported body mesh, head mesh, and animation files into a new Blender session, to double-check whether they all have the same bone name setup.

If you're using (a) DMX export(s) for the mesh, make sure that no mesh object in Blender has the exact same name as a bone on the armature.

Would you be willing to share a Blender session of the to-be-exported model? This seems like something that's probably hard to debug without specific examples to test with.
