There was just a huge ban wave in August, if you missed it and also read all the different threads being created in this forum, VAC is working fine.

Report cheaters ingame or thru the profile and move on, there is nothing else you can do.

I follow the VAC forum daily or whenever possible, I stayed on top of the ban wave. But it seems that it is bad and each day the number of players using hacks on the servers gets worse. It seems that they banish so much, but nothing changes.
And yes, I also use an in-game report system.

As far as "what happened", with respect to cheating in an online multiplayer game with strangers on the internet, the answer is :

Nothing happened.

It's always been like that, sadly, and might always will be like that. Cops exist also.

Does that mean that there will not exist criminals or would be criminals ? Same logic.

Maybe you missed all the locked threads on the first page before doing your research, and before posting this here.

Also, Overwatch has absolutely nothing to do with VAC, and you mentioned Overwatch at the end of your OP above :
