Xbox one controller
I connect my controller to my pc using a usb, the light turns on and works completely fine, I am able to run around Skyrim for a time and then it randomly becomes unresponsive. Whatever I was doing at the time is continued (if I was running and turning slightly left I would continue to do so indefinitely). the only fix I have mustered up is to unplug and plug it back in. I have tried both my bundled controller as well as my purchased controller and they both seem to have the same problem. Any ideas?

Plug in into a USB port directly; no Hub

Check the batteries.

Q: So can i use my XB1 controller and hook that all up and play just by itself??? Or do i need the Usb/Usb wireless adapter hookup also??

Yes, if it is a Microsoft branded one those support both Wired and Wireless.
To use it Wired on a system such as Windows-PC, simply plug it in via MicroUSB-to-USB cable.

If not using it Wirelessly w/ Rechargeable batteries, remove the batteries.
