Very much this. International shipping of physical goods can take weeks on end.
It's not that people don't understand. It's just that people with crummy Internet aren't really the target demographic for digital distribution services. And your ask is, "create a system that generates a lot of overhead, and cost" How many thumb drives do you figure they'll need? Plus they'll need warehouses where people will load the games onto the thumbdrive and mail them out. What do you imagine the fee is? And what if it's more like $30? Is the expectation for Valve to break even or take a loss because servicing people with crummy Internet supersedes all other concerns?
I mean in my youth I lived in the middle of know where, dialup Internet was all there was going to be for years. But the Internet wasn't going to stop for my slow connection. I really noticed it with Unreal Tournament 2003, where 20-30MB map downloads effectively excluded dialup users. So you either waited for the future to arrive in podunk u.s.a., or you moved to a place that has the infrastructure that meets your requirements.
I empathize with crummy Internet, but it's not really Valve's problem to solve. Good enough Internet is part of the system requirements. If you don't meet them, you don't meet them. That's your end to deal with, same as if you were running an unsupported OS, or a system to slow to play the games you want.
