Moderator actions are entirely under moderator's own discretion (within their own interpretation of the rules) so they can choose to not take action against an account, permanently ban it outright or anything in between irregardless of previous infractions.
Yes, the hub moderators can see and review all previous infractions account has made (or rather all warnings and bans issued against it) within the hub (Steam moderators and support can probably view all infractions within Steam).
Do note that account's post history, at moderator's own discretion, may also be taken into account when considering severity of the punishment.
I just got a 24 hour ban for "off-topic post" because I made an on topic post but in the form of analogy.
I appealed it. Steam support replied with a copy paste response about how they investigated their actions, and found that they did nothing wrong. I appealed it a second time, but during normal American business hours. I got a human typed response, and it said that the ban had already expired, but was misapplied, and would be removed from my record.
This is the second time this happened in this exact same manner:
Nonsensical ban -> appeal -> copy-pasta response saying to piss off because ban is legit -> 2nd appeal during American business hours -> human response stating ban was misapplied and will be removed from my record.
I can only assume that they outsource after hours and weekend moderation duties.
Yeah, the moderation at least on the forums is outsourced afaik. Support may actually be people at Valve which explains the discrepancies between what the moderators deem bannable offenses and what Support deems bannable offenses.
These days, I really only post trying to help people with technical or account security issues, so I haven't really gotten any bans in a while. But I have friends who use the forum more frequently than I do and actually participate in discussions/threads and I've talked at length with them about the state of moderation at the moment. They've been subject to nonsensical bans on more than one occasion and very quickly were able to get them appealed by Support.
So I personally believe it may very well be 2 different sets of people.
Usually when you challenge them and tell them they are censoring tyrants

Yes, moderation on steam platform is horrible. Too much power is in the hands of people who dont have the mental capacity to judge.
There should be more strict audit trail visible for anyone who is interested in it.
e.g. moderator can hide posts or ban users, but the reasons and trace can be viewed by anyone, thus giving the "judge" at least some kind of mental factor against him going full ape.
Well, yes. Attacking and insulting a moderator in response to being moderated is one easy way to get your warning/temporary ban escalated into permanent ban. Answering politely and respectfully on the other hand might get the ban lifted sooner.
for the steam forums its eu moderation who has no business handling moderation banning based on false flagging reports from other eu ♥♥♥♥♥♥♥♥ abusing the system.
its been like 10 years the users who abuse the report function should almost always be using the block function and moderation dont even bother trying to deal with false flagging because they agree with more often then not the politcs of the user doing the false flagging and abusing the report function.
there will be a post its obvious rage bait from the left. people will report it for obvious reasons. 3 people who no longer find the "joke" funny will respond the rage baiter will abuse the report function with out gerifers and moderation does no documentation to even try and deal with it.
virtually no one whos getting banned is actually breaking rules anymore.

-4 i need more context this make no sense at all this person is milk toast and the people showing replies are actually trying to piss him off
-4 or 5 this is obviously a false flag anyone using the forums knows what happened
-2 that should probably be banned more often but never get banned and have admitted to their alts they abuse being banned
