So I have two issues:
1. Inputs will randomly happen twice. This can be anything from attacks, movement, even navigating up and down in the menu, the button press will happen twice. Doesn't happen with every input nor is there a set pattern, but it is supremely irritating, especially when trying to navigate menus. This started happening immediately when first booting up the game.
2. When I got to stage 9 my controller just started vibrating at full blast. Occasionally it would vibrate less for a few seconds when beating up enemies but would shortly continue to vibrate violently. Only started happening at stage 9 and only starts when booting up the stage, not on the menus or anything. Quitting the game does not solve this issue, I need to exit steam fully in order to stop the vibration.

Neither of these issues persist in other games, so it must be a SOR4 issue. I am using a Dualshock 4 directly wired to my PC.
