And one of those mods which I strongly advise anyone to not even bother trying it[www.nexusmods.com] because it's unfinished and still very unrefined with heaps of major bugs alongside with it.
I save just after solving the Bridge puzzle in Peruvian Jungle - when I reload that save there are TWO Jonahs, until you go into the cave with the stele. Every time. I'm gonna upload a screenshot.
The other one is in the Path of Battle tomb in the Wild Jungle. Every time I get to the puzzle proper, the FOV goes doolally and Lara has to die to Continue the game. Every time.
The game used to crash after escaping Cenote, where Lara climbs out of the water, but a GPU with more RAM solved that one.

Are you using headphones? If so, does it run through a software? Maybe check there.
Well, the game never crashed me upon that hidden and around 1/2 of it underwater Tomb to unlock one of her skills. Probably I was just lucky, not sure (since imho this is probably the buggiest of the latest Tomb Raider trilogy and probably because it was developed by the Eidos company and some asorted dev' team because the one that made Rise of the Tomb Raider's was busy doing some other game during that time).
Although I keep encountering a bug that creates a duplicate corpse shortly after she gains that green "Mud covering herself to blend in better" skill right at the end of the Peruvian Jungle and when you stealthily kill a foe from a hanging below position. That one always creates me a double corpse (and many times preventing me to raid at one of those). Silly, I know how that one has escaped the Shadow of the Tomb Raider's (SotTR's) "Quality Assurance" team, but it's still there even after all these years.
Great tip. Thanks. I was changing the language from the game main menu to something else, then back to English but the fix didn't last long. Resuming the game after exiting brought the loss of dialogue issue back again. Doing it from the Steam properties setting looks like the fix holds. So far, so good anyway. :)
Edit:
The fix by changing the language and back from the Steam game properties looks like the best fix. Pretty bizarre, but there you go.
Further update.
Lost voice audio again! Oh well, time to switch languages again.
