Oh I've had a few drops from slain enemies/bosses drop to close to walls or on top of the "next level" staircase making them unattainable. I also had a few quest rooms with "kill all enemies in the room" then one enemy was walled off by room debris or barricades making it hard or impossible to get to the enemy.
1. There are no indicators of the weapon ranges, so the player is forced to learn them by trial and error. Also it was my intention that the player has to face the direction he or she wants to attack before actually attacking. It's all to boost the difficulty of the game and I'm completely honest when I say this. I know the controls might seem weird and/or difficult, but I chose to make them this way.
2. The issues you described in the second paragraph however are obviously bugs and I am working on eliminating them. :)
