same! i notice this happens when i only half done the tare down then save and quit. i come back in and notice 0 of 2 when i took all the stuff already and got 1 of 2. weird glitch for sure. now i have 3 unfinished and a rating of 4.9 :( and i didn't even do anything wrong.

This just happened to me with the first ever tier 5 contract. After accepting the contract the button was initially green, so I could have immediately finished it for $0. I didn't do that and instead completed all objectives, only to see that now the button has turned red.

it seems to be as soon as you upgrade to the tier 5, you do everything right and finish the job but the contracts are failed.

Well the good news is that even though the button is red and you have to confirm twice that you'll lose reputation, it still counts as a successful contract and awards five stars. So it's really just a display issue, at least in my case.
