Unlocking I am Ugh-Zan difficulty

So I just finished the First Encounter on Serious difficulty from the beginning. However, I dont see the new difficulty options if I wanna play a new game. Why it did not unlocked? How can I unlock this difficulty option so I can get the achievements?

I think Serious difficulty is supposed to be enough, but it seems to be bugged or something. I'm not sure if you're supposed to beat both TFE and TSE, but I think even that didn't work for me. Press F1 to open the console and enter this:

/sam_bUghZanActivated=1

I can't see the mental difficulty as well.

Do I need maybe to finish the Second Encounter as well on serious?

It seems like you just completely skipped everything I've said. Again:
The difficulty settings seem to be bugged. Use the console command.

Thank you. I totally got your point. I just wanted to reply to this fella. One more question. Will this console command affect the achievement?
